4 Introduction “This document summarizes the impact and significance of my contributions in [insert your focus].” Briefly summarize accomplishments in research/scholarship, teaching, service, & practice. Brief

Plan ahead of writing “Umbrella” or focus under which your work falls, e.g., Musical cues to remind nursing staff to turn residents every 2 hours. Development of same day heart failure clinic, impact on readmissions Evaluation of flipped classroom, improved student satisfaction and test scores

Identify your major contributions “My major contribution is in preventing readmissions to...” Outline them 1st with notes about evidence to support

Then write a paragraph for each major contribution (from your outline) End with summary of your planned next steps

Brief descriptions of the studies, practice innovations, teaching innovations Examples: In a series of studies, I documented… The practice model I developed and use in the clinic is based on collaboration among providers…

What new directions does the work suggest for practice/teaching? State findings in clinical terms, learner outcomes This intervention indicates that nursing homes can expect 30 fewer falls/100 beds annually. With this simulation students can significantly improve their observation and assessment skills. What products or artifacts have I created? Dissemination products: Journal articles, book chapters, books, invited lectures, consultations, presentations, etc. Teaching materials, websites, videos Who has a used my work? How many hospitals now use my clinic model? How many people have cited my work? How many faculty have adopted my teaching approach?
