4 Test Security Policies and Procedures
Florida State Board of Education Rule 6A , F.A.C., was developed to meet the requirements of the Test Security Statute, s , F.S., and applies to anyone involved in the administration of a statewide assessment. The rule prohibits activities that may threaten the integrity of the test. See the test administration manual appendix for the full text of the Florida Test Security Statute and State Board of Education Rule. Please remember that inappropriate actions by school or district personnel can result in student or classroom invalidations, loss of teaching certification, and/or involvement of law enforcement.

Examples of prohibited activities include the following: Reading or viewing test content before, during, or after testing Revealing the passages or test items Copying the passages or test items Explaining or reading the passages or test items for students Changing or otherwise interfering with student responses to test items Copying or reading student responses Causing achievement of schools to be inaccurately measured or reported

7 Test Security Policies and Procedures (cont.)
School personnel and proctors must read and familiarize themselves with the statute and rule in the test administration manual. The security of all test materials must be maintained before, during, and after test administration. The Test Materials Chain of Custody Form must be maintained at all times. No more than three people should have access to the locked storage room. Secure materials must never be left unsecured and must not remain in classrooms or be taken off the school’s campus overnight.

Any monitoring software that would allow test content on student computers/devices to be viewed or recorded on another computer or device during testing must be turned off. Test administrators must not administer tests to their family members. Students related to their assigned test administrator should be reassigned to an alternate test administrator. In addition, a student’s parent/guardian should not be present in that student’s testing room. Students and unauthorized school personnel are not permitted to assist in preparing secure materials before testing or in organizing and returning materials after testing.

Electronic Devices and Breaks Determine your school’s policy for the storage of electronic devices (including smart watches) during testing. Direct test administrators on what to do if students have electronic devices in their possession before testing begins. Ensure that test administrators are aware of the policy that students are NOT allowed to access electronic devices at any time during a test session, including breaks. If a student accesses his or her electronic device(s) during testing or a break, his or her test must be invalidated. Ensure that test administrators are aware of how to secure a student’s computer or device during a break. For short breaks (e.g., restroom), it is recommended that a visual block be applied to the student’s computer screen or device. For longer breaks, it is recommended that the student pause or exit the test. Students will have to resumed after the break to continue testing.

Admission of Students to Testing Precautions must be taken at testing sites when students are unknown to the test administrator or other school staff. For security purposes, photo identification, such as a Florida ID or school ID, must be checked before admitting unfamiliar students to a testing room. Proctors To ensure test security and to avoid situations that could result in test invalidation, FDOE strongly discourages testing students in large groups (e.g., in a cafeteria or an auditorium). If students are tested in a large group, the appropriate number of proctors must be assigned to the room to assist the test administrator.

Proctors (cont.) School personnel and non-school personnel may be trained as proctors. Prior to testing, proctors must be informed of their duties and of the appropriate test security policies and procedures. School personnel proctor duties may include preparing and distributing secure materials (e.g., test tickets). Non-school personnel may assist test administrators during test administration; however, they may not participate in any of the test administration procedures (e.g., distributing and collecting secure materials, providing accommodations).

Proctors (cont.) Volunteers (e.g., parents, retired teachers) may be trained as proctors and may perform non-school personnel duties. All proctors may help monitor rooms during test administration; however, they may not assist in rooms where their family members are testing. Each proctor who monitors a testing room for any length of time must sign a 2018–2019 Test Administration and Security Agreement and the Security Log for that room. These forms are located in the test administration manual.

School administrators, school assessment coordinators, test administrators, and proctors must receive adequate training prior to the test administration. ALL personnel must sign and return a Test Administration and Security Agreement stating that they have read and agree to abide by all test administration and test security policies and procedures. Including any other person who assists the school assessment coordinator or test administrator must sign and return a security agreement. Technology coordinators must also sign a security agreement form. Test administrators must also read, sign, and return a Test Administrator Prohibited Activities Form.

15 Paper-Based Materials Policies and Procedures
Chain of Custody The purpose of this form is to track test materials at all times, including their location, the dates and times they are handled, and the names of the people performing various activities involving the materials. The Test Materials Chain of Custody Form must be maintained at all times for secure materials. Schools must retain electronic or hard copies of completed forms for their files after materials are packaged for pickup and return the originals in the District Assessment Coordinator (DAC) ONLY – Administrative Records (AR) boxes.

Security Numbers All regular print test and answer books, test books, special document test materials (large print, braille, and one-item-per- page), and Reading and Writing Passage Booklets are secure documents and must be protected from loss, theft, or reproduction in any medium. A unique identification number and a barcode are printed on the front or back cover of all secure documents. Schools must maintain test security by using the security numbers to account for all secure test materials before, during, and after test administration until the time they are returned to the contractor. The test administrator should also maintain a record of the security numbers for all test materials assigned to him or her.

If a test administrator receives materials that are not already listed, the security numbers of those materials must be added to this record with the names of the students to whom the test materials are assigned. The security number(s) of the document(s) assigned to and returned by each student should be recorded and verified at the completion of each day of testing.

Caveon Data Forensics The FDOE uses Caveon Test Security to provide its Caveon Data Forensics™ for all statewide assessments. Caveon analyzes data to identify highly unusual test results for two primary groups: Students with extremely similar test responses; and Schools with improbable levels of similarity, gains and/or erasures. Flagging only the most extreme results.

19 Test Irregularities and Security Breaches
Test administrators should report any test irregularities (e.g., disruptive students, loss of Internet connectivity) to the school assessment coordinator immediately. A test irregularity may include testing that is interrupted for an extended period of time due to a local technical malfunction or severe weather. School assessment coordinators must notify Student Assessment & Educational Testing (SAET) at of any test irregularities that are reported.

20 Test Irregularities and Security Breaches
Test administrators should report possible breaches of test security (e.g., secure test content that has been photographed, copied, or otherwise recorded) to the school assessment coordinator immediately. If a security breach is identified, the school assessment coordinator must contact SAET immediately at If an electronic device was used to post or share test items, ensure that the information is immediately erased from the device and contact SAET at For all security breaches, a written report and supporting documentation must be submitted to SAET within 5 working days.

21 Test Invalidation Policies and Procedures
Remember that the main purpose of invalidation is to identify when the validity of test results has been compromised. Test administrators should discuss any situation involving possible invalidation with the school assessment coordinator, and the situation should be investigated immediately. For assistance identifying circumstances when invalidation is an appropriate course of action, review the guidelines in the test administration manual. For large group invalidations, contact SAET for guidance at

22 Paper-Based Materials Policies and Procedures
Missing Materials Within 24 hours of receiving shipment, verify that all secure materials are received and report any mispackaged or missing materials to SAET at immediately. School staff responsible for handling secure materials are accountable for the materials assigned to them. Investigate any report of missing materials. If, after a thorough investigation, a secure document is not found, the school assessment coordinator must contact the SAET at For missing materials, a written report and supporting documentation must be submitted to SAET within 5 working days.

23 Paper-Based Materials Policies and Procedures
Hazardous Materials If a used test and answer book is soiled (e.g., with blood or vomit), the SAC must Include: Student name, student grade, subject test, soiled secure book #, and explanation of incident At the school discretion, school personnel may transcribe responses into a replacement test and answer book. The damaged test and answer book should then be destroyed or disposed of in a secure manner (e.g., shredding, burning). DO NOT RETURN soiled test and answer books.

24 Communicate the Following Testing Policies to Parents/Guardians and Students
Electronic Devices Policy—Any student in possession of electronic devices at any time during testing or during breaks will have his or her test invalidated. Testing Rules Acknowledgment—All FSA tests include a statement that students sign verifying that they will abide by the testing rules. Leaving Campus—If students leave campus before completing a test session (for an appointment, illness, etc.), they will not be allowed to return to that session. Working Independently—Students are responsible for doing their own work during the test and for protecting their answers from being seen by others. If students are caught cheating during testing, their tests will be invalidated.

25 Communicate the Following Testing Policies to Parents/Guardians and Students
Discussing Test Content after Testing—By signing the Testing Rules Acknowledgment, students also affirm that they will not “discuss or reveal details about the test items or passages after the test. This includes any type of electronic communication, such as texting, ing, or posting online, for example, on Facebook, Twitter, Snapchat, or Instagram.” While students may not share information about secure test content after testing, this policy is not intended to prevent students from discussing their testing experiences with their parents/families.

27 Students To Be Tested All students enrolled in tested grade levels/subjects participate in the Spring 2019 FSA and NGSSS administration with or without accommodations, per Section (3)(a), Florida Statutes (F.S.). Students must take the test(s) appropriate for the grade level/subject in which they are receiving instruction. FSA ELA Grades 4-6, all students must take the same grade-level test for both the Writing and Reading to receive the FSA ELA score

28 Students To Be Tested: ESE and ELL
Students with Disabilities (ESE) Students with disabilities participate in the statewide assessment program by taking one of the following: FSA/NGSSS without accommodations, FSA/NGSSS with accommodations, or Florida Standards Alternate Assessment. All determinations regarding participation in the statewide assessment program must be documented in the student’s IEP or Section 504 Plan. English Language Learners (ELLs) All ELLs participate in statewide assessments. Students who are identified as ELLs (Levels 1-4) must be provided with the allowable accommodations listed on Appendix A of the manuals.

Weekly Briefing #23796 Grades 3–5 ELA and Mathematics: Elementary schools identified for calibration have been selected for all tests (ELA Writing, ELA Reading, and Mathematics). Grade 3 ELA Reading and Grades 4–5 ELA Writing will be returned early on April 8. Make-up testing may be administered until April 12. Grades 4–5 Reading will be returned early on May 3. Grades 3–5 Mathematics will be returned early on May 10. Make-up testing may be administered until May 14. Grade 6 ELA and Mathematics: If Grade 6 is included in an elementary, K–8, or K–12 school, all Writing, Reading, and Mathematics materials for Grade 6 will be returned with other grade levels as indicated above. 42 General Information about Accommodations
Appendix A of the manuals provide information concerning allowable accommodations for students with disabilities and for ELLs. For eligible students participating in the FSA and Science paper-based assessments, the following accommodations are available as indicated in the IEP or Section 504 Plan: Large print, UEB Contracted braille, UEB Uncontracted braille, One-item- per-page Instructions for administering large print and one-item-per-page accommodations are found in Appendix A of the manuals. Scripts and instructions for administering braille accommodations are provided with braille test materials and are available on the FSA Portal for Writing, Reading and Mathematics and via Avocet for Science. For students taking an FSA ELA Writing test using a flexible responding accommodation to provide a typed response, transcription of the response into the paper-based test is NOT required. 44 FSA PBT Test Materials All ELA (Writing and Reading) and Mathematics assessments for Grades 3–6 will be administered on paper. 45 FSA PBT Test Materials FSA Security Numbers
A unique identification number and barcode are printed on the front cover of all secure test and answer books. The security number consists of the last eight digits of the identification number located next to the barcode. Schools must maintain test security by using the security numbers to account for all secure test materials before, during, and after test administration until the time materials are returned to the contractor.

46 FSA Test Materials FSA Writing Planning Sheets
All students taking ELA Writing receive Writing Planning Sheets that they may use to take notes and plan their responses. Students testing on paper will sign the Testing Rules Acknowledgment printed inside the test book and may disregard the statement on their planning sheets. Planning sheets are distributed to students at the beginning of the ELA Writing test session. Ensure students have enough desk space to use their planning sheets. Planning sheets will not be scored. Responses must be transferred to the test and answer book within the allotted time. Used planning sheets are considered secure materials and must be kept in locked storage. Return in the District Assessment Coordinator (DAC) ONLY- Student Work Record (SWR) boxes.

FL Statewide Science Security Numbers The security number consists of a nine-digit number followed by a check digit. The packing list will indicate the number ranges assigned to each school. The Science test books have a security number, answer books do not. The security number is on the back of the test book. Test book cover also have a test form number (upper left side) that must be gridded on the answer book.

49 Science Test Materials
Periodic Tables (Grade 8 Science) Stand-alone pages, shipped with test materials. Test administrators will distribute periodic tables on the day of the test, according to the directions in the scripts. Students may refer to them at any time during both sessions of the test. After students complete the test, the test administrator should collect all periodic tables. Used periodic tables are secure materials and must not be reused. Return the used periodic tables in the District Assessment Coordinator (DAC) – SWR ONLY boxes.

Four-Function Calculators (Grade 8 Science) Four-function calculators must be provided to students for Grade 8 Science. Schools should have an adequate supply of four-function calculators. Schools can order additional four-function calculators at Ensure that one working calculator is available for each student in Grade 8 for use on the Science test. Calculators may not be used by students on the Grade 5 Science test.

51 Required Test Materials (FSA and Science)
Security Logs An accurate Security Log, provided in Appendix D, must be maintained in each testing room. Anyone who enters a testing room for the purpose of monitoring the test is required to sign the log. This applies to test administrators, proctors, and anyone who relieves a test administrator, even for a short break, regardless of how much time he or she spends monitoring a testing room. Seating Charts Test administrators are required to maintain an accurate seating chart for each group of students in their rooms during testing. Ensure that test administrators record all information and that they create a new seating chart if the seating configuration changes during a test session. Create a new seating chart for make up sessions.

56 Test Invalidation Policies and Procedures
Do Not Score (DNS) and UNDO Bubbles DNS and UNDO bubbles are located in the SCHOOL USE ONLY box on the student demographic page. The DNS bubble must be gridded when a test and answer book is invalidated. If a DNS bubble has been gridded by mistake, erase the DNS bubble and grid the UNDO bubble. PBT CBT The Test Delivery System (TDS) contains a new scientific calculator (Florida specific Desmos) which allows for better zoom and color contrast functionality. A scientific calculator is only available online for Sessions 2 and 3 of Grades 7 and 8 Mathematics and for Session 2 of the EOCs. Handheld calculators may be provided to all students during Session 2 and (if applicable) Session 3 only. Students using paper-based accommodations must be provided an approved handheld scientific calculator during the approved sessions. Providing a calculator with prohibited functionality or in the wrong test session (Session 1) is cause for test invalidation.

PBT CBT TestNav has a built-in four-function calculator. Students may use the built-in calculator, or they may use a handheld four-function calculator. Reference sheets are provided for the Grades 7–8 FSA Mathematics and FSA EOC assessments in TDS for computer-based testing. Schools may provide hardcopy reference sheets to ALL students taking the test. Any hardcopy reference sheets must be printed or copied from the files found on the FSA Portal. Schools may provide each student with a new copy of the reference sheet for Session 2 on Day 2, or they may return the same hardcopy reference sheet from Session 1 for students to use during Session 2. After students complete the test, test administrators must collect all reference sheets. Used reference sheets are secure materials. The school assessment coordinator should package all used reference sheets in the DAC ONLY –SWR boxes.

The Periodic Table of the Elements for the Biology 1 EOC Assessment is provided in a pop-up window for computer- based testing (paper copies are provided with paper-based accommodations). Schools may provide hardcopy periodic tables to students taking the Biology 1 EOC. Any hardcopy periodic tables must be photocopies of the perforated page in Appendix D of the manual. Used periodic tables are secure materials and must not be reused. The school assessment coordinator should package all used periodic tables in the DAC ONLY –SWR boxes.

114 FSA Headphones/Earbuds
CBT Students must have headphones or earbuds for both sessions of ELA Reading (Grades 7-10 and Retake). If any students are taking an accommodated form with text- to-speech, ensure that they also have headphones or earbuds. Headphones or earbuds should be checked to ensure that they work prior to the first day of testing. Plug headphones or earbuds in and adjust system volume prior to launching the secure browser on each day of testing. During a built-in sound check, students will verify that headphones and earbuds are working prior to entering the test.

115 FSA Test Tickets CBT Each student must have a test ticket to log in to computer- based FSA assessments. Test tickets are generated in TIDE and contain a student’s login information. Test tickets will be printed by school assessment coordinators and will be distributed to test administrators on the day of testing. Test tickets are considered secure materials and must be stored in a secure location before and after testing. Retain at school for one year. Note: Students must verify that the student and school name is accurately displayed on the computer.

Schools may use secured tablets for testing in Spring 2019, in accordance with District guidelines (refer to Weekly Briefing # 24187). Attachable keyboards are required for test administration, along with headphones for applicable sessions.  All students to be tested on the tablets should complete the practice test on the tablet, to ensure comfort with the device. Mobile technologies (laptops and/or tablets) must be fully charged and activated to allow for installation of updates in advance of the testing window. In addition, ITS must certify the specific setting, and the appropriate infrastructure trial must be conducted using the devices in place.

Review the instructions and information for technology coordinators (located on the FSA Portal and PearsonAccess Next), AND test administration and security policies and procedures included in the manuals. Verify with your technology coordinator that the most recent version of the secure browser is installed on all testing devices. Ensure that the technology coordinator has verified that all computers and devices meet the requirements. Create a plan for handling technical issues during testing. The technology coordinator should also be involved in all planning meetings to provide input on logistics and resolve any network issues.

PBT CBT Prepare Test Settings, Computers, and Devices Tests should be administered in a room that has comfortable seating, good lighting, and an appropriate temperature. Make sure that testing rooms are adequately ventilated and free of distractions. Sufficient work space should be provided for students to use worksheets, work folders, planning sheets, reference sheets, and passage booklets, as applicable. Check the configuration of the testing rooms to make sure students will not be able to easily view other students’ test materials or computer/device screens. If necessary, use visual blocks (e.g., file folders taped to the sides of computer screens). Also, check for and remove all unauthorized visual aids posted in classrooms or affixed to student desks.

CBT Prepare Test Settings, Computers, and Devices (cont.) Outside each door to the testing room, post a sign that reads TESTING—Please Do Not Disturb. A watch or clock is required to ensure the correct amount time is provided. Display starting and stopping times as instructed in the test administration script. Before each test session, test administrators should launch the secure browser on each computer or device. Choose Option A, B, or C to advise students what to do when they have finished testing when administering the Reading, Mathematics, or EOC. Provide instructions for how to handle these circumstances: A student has not participated in a practice test session. The practice tests contain sample test items to prepare students for the computer-based assessments and will help familiarize you and your students with the CBT tools, as well as the process for responding to items. Monitor each testing room to ensure that test administration and test security policies and procedures are followed. Make sure that Security Logs and seating charts are being completed properly and that all required administration information is being maintained in each testing room. You and the technology coordinator must be available during testing to answer questions from test administrators and to assist with technical issues. Test administrators should contact you or the technology coordinator if an error message appears on a student’s computer screen or device during testing and he or she cannot resolve the issue.

CBT Supervise Test Administration and Maintain Test Security (cont.) If the Internet connection is interrupted during computer-based testing, students will not be able to continue testing. If the Internet connection is not restored in time for students to complete the test, the test administrator should contact you or the technology coordinator for assistance. If a technical issue interrupts testing and is not able to be resolved quickly, you should contact the FSA Help Desk at or Pearson Customer Support at and notify SAET at immediately. Note: If a student is unable to complete a session due to a technical issue, FDOE will only grant approval for the student to resume testing the following day, if the technical issue was reported in a timely manner.

CBT FSA: Submit Re-open a Test Session and Re-open a Test requests in TIDE for students who need to return to an FSA test session after mistakenly submitting it (same day only). Contact SAET at or via for assistance with processing these requests. Which requests to use in different scenarios: Re-Open a Test: re-opens a test that a student mistakenly submitted early. The student’s test must be in Completed, Submitted, or Reported status. Re-open a Test Session: re-opens a test session when a student inadvertently starts an incorrect test session. Requester will indicate which session should be opened. Restart a Test: Allows the student to restart a test from the beginning. This request deletes all student responses. This request can be used if a student begins the incorrect test and that test should not be reported. Must be approved by FDOE. Restore a test that has been Restarted – Allows the student to revert to the original test opportunity from before the test was restarted. Must be approved by FDOE.

CBT FSA: In TIDE, monitor student progress and test completion rates for CBT. In TIDE, use Participation Reports to track completion rates and determine which students still need to be tested. Further information on Participation Reports can be found in the TIDE User Guide. NGSSS: The Proctor Caching computer must be on, and the Proctor Caching software must be active during testing. Start Test Sessions—A session must be started in PearsonAccess Next before students can begin the test. Monitor Session Status—Keep track of each student’s testing status in PearsonAccess Next. Resume Students’ Tests—Resume tests for students who have exited (intentionally or unintentionally) and will finish later. Undo Test Submission - If a student submits his/her test and needs to return to the test the same day, contact SAET at

PBT CBT Schedule and Supervise Make-Up Test Administrations Ensure that all test security and test administration policies and procedures are followed while conducting make-up tests. Be available to assist test administrators as needed during make-up test administrations. For all FSA assessments, sessions must be completed in the designated order. For example, Session 1 must be completed before Session 2. For FSA Math assessments with three sessions, students must complete Session 1 and Session 2 before completing Session 3. Two- or three-session tests must be administered over two days (with Session 1 administered on Day 1 and Sessions 2 and 3 administered on Day 2 for three-session tests). Students with extended time accommodations may test one session per day. Any students absent for a session may not participate in the next session until they have completed the session that they missed.

PBT CBT FSA: Update Student Information in TIDE If student information is discovered to be incorrect during testing, update the information in TIDE immediately following test administration. Invalidate tests, if needed. You will need the student’s FLEID number and the reason for invalidation. Refer to the TIDE User Guide. Invalidations must be entered by the last day of the test administration window. FSA and NGSSS: Record Accommodations Ensure that all accommodations provided to and used by students are recorded on each record of required administration information, as applicable. This documentation may be necessary in the case of investigations regarding possible test irregularities.

CBT NGSSS: Conduct these activities in PearsonAccess Next by the last day of the test administration window (May 29). Mark a Student’s Test Complete If a student exits and will not resume testing or if a student accidentally exited a test instead of submitting, you must manually mark that student’s test complete. Stop Test Sessions All students must be in Completed or Marked Complete status and any Ready students must be removed to MANUALLY STOP a test session. Invalidate Student Tests Any test that should not be scored must be invalided. Students must be in completed or marked complete status to invalidate a test. Update Student Information Edit demographic fields once a student is in Completed or Marked Complete status. Refer to the list of fields in the PearsonAccess Next User Guide.

Recount books one last time as you place them in the box. Pack multiple grade levels or subjects in the same box, however, keep material organized by grade levels or subjects. Ensure that all test books are packed either face up OR face down. Do not place a TBS label and an NTBS label on the same box. Each box must only contain one type of material (TBS or NTBS). Do not place multiple TBS and/or NTBS labels on a box. Each label has a unique barcode number that is used to record and track your materials by both TDC and the vendor. Placing multiple labels makes it difficult to track your box.

TDC POLICIES: All testing materials must be securely stored at the school site. Please refrain from picking up materials on your way home. When requesting orders through TDC Documents’ “Order Forms,” please be advised that orders placed before 3:00 pm will be available for pick up within 24 hours. Otherwise, allow 48 hours for processing. Notification s will be sent only if materials are on backorder. All testing materials must be inventoried and discrepancies reported to TDC within 24 hours of material delivery. ONLY TDC ID card holders are authorized to receive or return materials. Staff must be assigned by the school principal through the Assessment Services’ “School Assessment Team Identification and Contact Information” screen.
