1 Crest Instructions for Staff Online Accident Reporting

2 Create New Incident You will need to complete your details in the Person Reporting section. You then complete the Incident Details, any box highlighted with a red asterix is mandatory and must be completed before the form can be saved. Please note that the Team box in this section is for the Injured Party. This button is not on the schools portal.

3 Create New Incident The types of incident you can choose from include:
Injury Accident, Near Miss, Verbal Abuse, Physical Assault, Motor Property Damage/Loss. The forms are dynamic and change depending on the choices you make and information you enter. You only need to report the details of an incident once and add the Person Involved here. You can add multiple people to the same incident e.g. a clash of heads incident at school.

4 Adding Persons Involved
There are some simple details to add for each Person Involved. If you don’t know the persons date of birth, just select today.
