5 Participation / Try-Out Requirements
WIAA Physical / Alternate Year Form Registration Form & Checklist Medical & Emergency Information Parent & Student Agreement Acknowledgement Form (Understand and will abide by the WIAA Eligibility Requirements and the Co-Curricular Code) $85 Athletic Fee ($425 family max) Payment of unpaid fines and fees Concussion Sign-off and understanding

6 WIAA Physical Card WIAA Alternate Year Card
WIAA Physical Form– must be signed by a physician in the U.S. Grades 9 & 11 Please note that the WIAA physical year begins on April 1st of 2017 and is good through the school year. Physicals done BEFORE April 1st, 2017 = need one for the season. WIAA Alternate Year Form– must be signed by a parent/guardian. Grades 10 & 12

7 Athletic / Activity Handbook
Enforced at all times, 12 months a year, 24 hours a day, 7 days per week, on or off school grounds Unbecoming conduct from a student involved in athletics/activities will not be tolerated– participation is a privilege, not a right Penalties/Consequences for Violations of the Athletic/Activities Code– page 11

8 Violations- Page 11 First Violation: Loss of 1/4 of regular season contests Second Violation: Loss of 1/2 of regular season contests Third Violation: Loss of one year from the date the decision is determined Fourth Violation: No longer be able to participate (Conduct Unbecoming: 1 contest/7 days)

9 Academic Eligibility/Attendance Policy
Students must pass all courses or have a 2.00 GPA during every marking period Consequence: 15 consecutive school days of no contests for winter and spring sports (21 days for fall, summer) Students must be in attendance for at least half of his/her scheduled classes in order to be eligible to participate in practice or a contest that day. Page 3 of Handbook

10 Collection Days For ALL sports, you must have your forms and fee submitted BEFORE the first day of practice in order to participate…the sooner the better! We hope to have online registration next year for all sports

11 Role of Parents/Guardians
Support your son or daughter – attend his/her games and compliment him/her for playing fair and to the best of his/her ability. Require your son or daughter to adhere to the Code of Conduct. Ask your son or daughter about his/her experiences and listen to what he/she has to say. If there is a problem, have your son daughter address the coach first. If not resolved, you can address the coach with your son or daughter. Then, if things are still not resolved you can address me/Athletic Director.

12 Role of Parents/Guardians
Support our coaches and plan vacations around the season if possible. Do not ridicule or yell at your son or daughter or his or her teammate for making a mistake or losing a competition. Respect the officials and their authority during games. Please do not question, discuss or confront coaches or officials at the competition site. Take time to speak with coaches at an agreed upon time and place.

13 Role of Parents/Guardians
Maintain your dignity under any circumstance. Participate in only positive cheers for our teams. Be a POSITIVE role model!

14 Request and Release for Private Transportation
If an athlete will not go to or return from a competition on district provided transportation, the athlete’s legal guardian must sign this form and give it to the coach. (No one else is allowed to sign this form.) All athletes are encouraged to ride team transportation to and from competitions.

ImPACT Concussion Testing All contact sports that have high risk of head injury Parent permission release for baseline tests Post test done through insurance by Froedtert Health Children’s Hospital Aurora- in progress

19 Athletic Training continued
Return to Sport Protocol Day 1: After symptom free >48 hours, light biking Day 2: Increased workout intensity, weights Day 3: Non-contact practice drills Day 4: Full contact practice with MD clearance Day 5: Cleared to play per MD If athlete presents with symptoms at any stage, athlete returns to previous stage.
