The Schedule tab provides access to all appointments that are entered, edited, and removed. In the daily or weekly tab view, double-click the time slot to enter an appointment for existing or new patients. To view information, select from the drop-down menu options. In the maintenance tab, click the field to add clinicians, locations, reason codes, and to add and edit schedule templates.

When you click Patient, the patient screen pops up
When you click Patient, the patient screen pops up. Click Add to add a new patient or double-click a patient’s name to access an existing patient’s information. The patient pane is divided into 8 tabs for entering information: Demographics, Insurance, Charge Ledger, Responsible Party, Appointments, Distributions, Contacts and ProtoCHART.

The patient screen defaults to the Demographics tab.
To enter or change information: Click Add or Edit at the bottom of the screen. Click any field to enter information. Click Save, Close or Exit when done. To add a Patient Note: Click Add Patient Note towards the bottom of the screen. Click the field to enter information. Click the Save, Edit or Cancel when done. To print the Patient Note: Click Print Patient Note. A window pops up where you can view the note before printing it. Click the printer icon in the toolbar. To enter Universal Referral Forms: Click URF. To create a new referral, click New. Enter the information in each field, click Save when finished. A window will pop up allowing you to preview the referral form. To print, click the printer icon in the toolbar. To cancel, click the red X in the upper right hand corner.

The Insurance tab allows you to view, add, edit and delete insurance information.
To enter a new provider: Click Add Ins. Enter the name in the name field then click on the Insurance Address field. A pop-up box will open for you to select the appropriate name and address. Highlight the correct option then click Okay. Click the field or choose from a drop-down list to enter the necessary information. When finished, click Save. To change existing provider information, Click Edit Ins or Delete Ins.

The Charge Ledger tab provides a list of all charges for a patient.
To add a charge: Click Add. To select to whom you would like the charge to be billed, select from the drop-down menu next to Bill To. Likewise, select the form type. Enter all charge related data. Click Post. To edit a charge: Click Edit then select to whom you would like the charge to be billed and the form type. Edit the data you want to change. ProtoMED will ask you if you want to rebill that line item. To remove a charge: Click Delete. Click Yes to confirm the deletion. To go to the last charge entered: Click Last. Modify any data necessary, including the date of service. Click on Post when finished.

Responsible Party tab No information can be entered or edited The Responsible Party screen is automatically populated from the demographic tab. No information can be entered or edited through this tab. This screen is automatically populated from the demographic tab

Appointments tab This screen shows all scheduled appointments
To sort information, click the field title The Appointments screen shows all scheduled appointments for the selected patient. To sort information, click the field title. This screen shows all scheduled appointments for the selected patient

Distributions tab The Distributions screen shows the distribution of payment between patient and insurance provider. This screen shows the distribution of payment between patient and insurance provider

To change information:
Contacts tab This screen shows contact information for: Patient Responsible party Other information The Contacts screen shows contact information for the patient, responsible party and other information. To change existing information: Click Edit. Enter the information into the appropriate field. Then click Save when finished. To change information: Click Edit. Enter information. Click Save.

The ProtoCHART is the electronic medical record builder
The ProtoCHART is the electronic medical record builder. This tab allows you to add, edit, delete, print and sign and post information to a patient’s chart. CHIEF COMPLAINT To enter the chief complaint: Click Add at the bottom of the screen. Click Chief Complaint and select boxes next to all relevant data. As you click new information, it appears in the box on the lower left of the screen. Click Save to enter, or Cancel. VITALS To enter vitals: Click Vitals and select boxes next to all relevant data. As you click new information, it also appears in the box on the lower left of the screen. Adjust the data by using the slider bars or entry box. Click Save to enter or Cancel. REVIEW OF SYMPTOMS To view and edit the list of symptoms: Click ROS. Double-click the condition to turn the radio button green, indicating the patient has that symptom. Double-click the condition again to turn the radio button red when the symptom is gone. You can add, edit and delete symptoms by clicking the appropriate buttons. PAST MEDICAL HISTORY To view and edit the patient’s past medical history: Click PMH. Double-click the condition again to turn the radio button red when the condition is gone. You can also add, edit and delete symptoms. FAMILY HISTORY To view and edit a patient’s family history: Click Family History. Double-click the condition to turn the radio button green, then select the relative to whom the condition is attributed.

SOCIAL HISTORY To review the patient’s social history: Click Social History. Double-click the item to turn the radio button green. Double-click the condition again to turn the radio button red when the condition is gone. You can add, edit and delete items. Click Save to enter or Cancel. PRESCRIPTIONS To view and edit prescriptions: Click Prescriptions to enter all medication information. Select from the list of drop-down options. LAB ORDERS To view and edit lab orders: Click Lab Orders to enter any lab requests. Click Save to enter or click Cancel. COORDINATION CARE To add or edit a patient’s coordination of care: Click Coordination Care. Enter patient referrals and/or transfers. You can type this in. Or you can also select from the drop-down menu. Click Save or Cancel. SUPERBILL To enter billing data: Click Superbill. Highlight the appropriate service from the column. To select more than 1 item in a column, hold the control key and click all additional items. Click Save. When finished, click Sign&Post to post the record.
