2017 Qualifications What has Changed
The qualification period will now be from January 2016 – June 30th This means your activities must be completed during this time frame and your forms must be submitted during this time frame. **Except for Employees/Spouses without a MAPS card for We will except physical exams from July 1st, 2016 – June 30th, 2016 The activity list and exam list has been combined for There is only 1 activity list for 2017. If you are re qualifying for a MAPS membership, you will no longer be required to complete 5 items but only required to complete 3 items. Spouses are able to attend Lunch & Learns, Wellness Fair, and Mid Day Wellness Events. Wake up for Wellness events are closed to Employees Only

2017 Qualifications What is new What does Self Reporting Mean?
Self reporting for qualifications What does Self Reporting Mean? We will no longer be accepting paper verification forms. You will be required to log onto the County’s Employee Resources Intranet Site> Wellness Website> and fill out a qualification form. Self reporting will move the Wellness Program to the honor system. Meaning we are trusting that what you report is accurate and true. If you have a spouse, you will be able to self report your spouse’s information. They will not be required to log on and report their wellness information. A random audit will be done at end of qualifications. We still have to ensure there are no fraudulent claims.

Self Reporting System If an Employee or Retiree has never logged on before to the County’s Employee Resource Intranet Site, they must register first before they can log on. If they have logged in before, they will bypass the registration step. Employee’s, Retiree’s, and Planning/IDC/Redevelopment/Transit all have different ways to register for the County’s Intranet site. Please make sure you hand out the correct packet to the employee. There is a total of 3 separate packets. Example: - Retiree packets will be mailed out by the Wellness Coordinator, however if they do call into your department refer them to the Retiree Packet. - Planning/IDC/Redevelopment/Transit employees will need to be referred to the Planning/IDC/Redevelopment/Transit packet. This is 1 packet for these departments, not 4 separate packets. All other employees (both Union and Nonunion) will need referred to the employee packet. All 3 packets are exactly the same except for the registration piece.

You must fill out all required fields (marked with an *) before you submit the document. Upon verification of your employment, the webmaster will send you an with your assigned user ID and password within 5 business days.

Note: The password you create will be used when logging into the Employee Resource Intranet Site. Upon verification of your retirement, the webmaster will send you a confirmation within 5 business days stating you have access to the Employee Resource Intranet Site.

On the next screen click Wellness Program. This will take you to the Wellness webpage.

Completing your Wellness Form
Self Reporting System Completing your Wellness Form You must fill out all required fields (marked with an *) before you submit the document. You may save your progress and return later to complete form before submitting. Please note, if you decide to upload any documents and save, when you return, you will need to re upload that document. The form can only be submitted ONE TIME. We will not accept multiple submissions. If you have an applicable spouse, they will also have to participate in the program for you to qualify for MAPS. You will be able to self report their information on the same form. - Last day to submit your renewal or new form will be June 30th, 2016

Auditing Process A random audit will be conducted at the end of qualifications. (July 2016) Each employee will be assigned a number after their documentation has been submitted. From there, the Wellness Coordinator and MAPS Committee will generate the random numbers for auditing purposes using random.org. RANDOM.ORG - True Random Number Service 10% of the employees who qualified will be audited (This is a very SMALL amount). If you fall in the audit and were qualifying for the first time, you will need to provide supporting documentation for the annual physical within 30days of being notified to Human Resources. If you were re qualifying and fell within the audit, you will need to provide supporting documentation on the one activity you had selected within 30days of being notified to human resources. If any fraudulent claims were reported within the wellness audit, you and your family will no longer be eligible for the MAPS program for that qualifying year, be subject to mandatory audit for the following year, and have audit findings placed in personal file. (This is not a write up, it is used only for the next years auditing purposes). **Please note, if there are any fraudulent claims, the audit percentage will increase by 10%

Suggestion: Still fill out the verification form and hang onto it. Then if you are selected for the wellness audit, you just have to turn the form in.

Wellness Timeline October –Advocate Training – Slide show is located on Wellness Webpage under> Advocate Resources November – First week in November – Receive 2016 MAPS Cards with 2017 qualification packet. Employees will only receive a couple packets. You will need to make copies for employees that will want one. Wellness newsletter will also be distributed. December – Q&A sessions for employees begin and will be ongoing till June January – Go Live, Pedometer Challenge Begins, Lunch and Learns begin again February – Advocate Appreciation Breakfast March – Wellness Fair
