2 Step 1 Medical Underwriting for an Informal Offer
Kai-Zen works best if client is rated Standard or Better with the carrier. Doing an informal medical review let’s you know up front and avoids client paying for a trust they cannot use if they don’t move forward with the plan.

3 Allianz Income and Net Worth Requirements for Kai-Zen Cases…
Income of $400K Or Income of $200K-$399K AND Net Worth of at least $500K Or Income of $100K- $199K AND Net Worth of at least $1MM Medically rated as standard or better Age *Please make sure your client fits the profile(s) above before submitting a case. Allianz will not reimburse your costs for ordering medical exam/APS’s unless case closes.

INFORMAL FORMS NEEDED TO SUBMIT CASE TO ALLIANZ DOWNLOAD. The forms below from Cover Letter for Allianz Informal Informal Sheet for Allianz Life DOWNLOAD. The forms below from Hipaa Form (NB3046-WS) *If the agent is not licensed with Allianz to write business in the state of Nevada they may submit the informal application, but MUST get licensed in Nevada PRIOR to submitting the formal application because the trust owning the policy is domiciled in Nevada.

Note* if the client is between years of age and is applying for more than 3Million in death benefit APS’s will be required

Face Amount – has to minimum of $1.5M for Kai-Zen cases – can be more What are considered “Liquid Assets” Cash Property immediately convertible to cash: Securities Notes Life insurance policies with cash surrender values U.S. savings bonds Account receivables Once this list has been complied for the prospective client total up the “liquid assets” and write the number, in the space provided. Please be sure and forward to and NOT to Allianz directly – they will not process unless it comes through Kai-Zen Plan/NIW first.

8 Submit Informal to get Medical Offer
The Cover Letter, Informal Cover Sheet and HIPAA should be submitted directly to for review. Kai-Zen Plan/NIW will review to make sure complete and client qualifies per Allianz requirements – then we will submit to Allianz. Please make sure lab slip and barcode are included or copy of exam – Allianz will not review an informal without the full exam/labs OR Medical Records.

10 Step 2 (For Allianz Only) Final Illustration and Kai-Zen Proposal for Client Your client got an offer of standard or better*….now you need the customized illustration and proposal to close the sale! Contact NIW for your customized illustration and Kai-Zen proposal. Please include Name, Gender, DOB, Death Benefit Amount and Medical Rating *In some cases Kai-Zen works for substandard medical ratings, contact NIW for special cases to see if your client would qualify

14 Allianz Application and Related Forms required for Kai-Zen Cases
For Allianz forms go to or work with your General Agent/FMO to obtain. State of Nevada application/worksheet form NB5056-R11, also know as Life Insurance Worksheet (the Kai-zen Trust is domiciled in State of Nevada - therefore agent must be licensed in Nevada PRIOR to taking a formal application) Producer’s Report – Form NB Personal Financial Questionnaire – Form NB2012P Replacement Packet – Nevada – Form NBAL0013-NV (if replacement is involved) See next few pages for sample “filled out” forms Other forms may be needed on a case by case basis

15 Kai-Zen Insurance Trust (example only) Page 1 Name of trust is normally the “Insured’s name” Insurance trust and insured is normally the settlor of the trust Please date the trust as you complete – all trusts MUST have a date or they are invalid Please make sure and complete ALL blanks including dates and initials at the bottom of each page

Settlor of the trust is NORMALLY the insured – unless the purpose is keyman or buysell Trust protector – normally for a REVOCABLE trust since the settlor is the insured – you do can waive – trust protectors are usually used for Irrevocable trusts

Most of the trusts for Kai-Zen are Revocable because the insured is looking to take income from the policy for supplemental retirement income – if trust is Irrevocable income taken out may be taxable Please make sure insured/settlor initials bottom of each page

John Goodwin 100 Contingent – Stacy Goodwin – Daughter This part tells the trustee how to distribute the death benefit once the insured passes away. If the insured/settlor wants to have a contingent beneficiary have them write PRIMARY next spouse and then CONTINGENT and the percentages below that

#ii at the top of the page should be the primary beneficiary Distributions – Living Benefits – who settlor wants the trustee to pay any living benefits and/or income to – normally this will be the insured/settlor Directs the trustee on how to pay any distributions – normally since insured is settlor and the trust is revocable – the settlor/insured would direct the trustee. Trustee would need sole discretion in example of Irrevocable trust
