With informal trainees you will collect Demographic Form Record of Use These two pieces of information are critical. Informal trainees are individuals who are trained via street outreach or are active substance users, low reading levels

8 Additional Required Forms
Every trainer must complete the Narcan Training and Distribution Excel forms. Informal trainings should be entered by the day they took place. If you completed 3 informal trainings on one day, you may enter all 3 trainings as one entry and do not need to enter each separately.

12 Trainee Data Collection Notes
We collect trainee “contact information” so that we may “follow up” with trainees after 3 months to see if they still feel that the training prepared them well for administering Narcan. Contact information is deleted once the follow-up survey is completed. We ask each person to create a unique identifier (ID) that they’ll use every time they (or you) enter data. It is not connected to an actual name of a person. This is used so that we can track Narcan use over time but still have people remain anonymous. Contact information and demographic information are collected at the beginning of a training. Post-test is collected at the end of a training Follow-up is collected 3 months post training (can be done electronically) Trainee data are typically collected using paper forms and entered into Qualtrics or the Excel file by a staff person

13 Narcan (naloxone) Training and First Distribution Form
This Excel form is completed by the trainer post training and summarizes the key training information Each training is one line of data – if two trainings are completed in one day, two lines of information are entered- one for each training (not necessarily each person trained). This form should be completed after every training and after you enter the trainee data.

17 Narcan (naloxone) Administration Form (aka: Record of Use Form)
Someone uses the Narcan. Then what happens? Sticker or flyer on every box or kit of PDO funded Narcan Unique local number for each community Individuals may report on-line and/or go back to the agency or person they got the Narcan from; the agency should collect Record of Use data at that time and provide more Narcan.

18 When someone wants to replace their Narcan….
We ask them if s/he would be willing to provide some feedback on their experience administering Narcan. If the person was trained originally, by you they should be “in the system” and they should expect to be asked questions about the experience because s/he will have already told them to expect it at the original training Everyone must go through a “consent” process before completing the Record of Use Form. The Record of Use Form can be completed on-line or on paper or verbally over the phone or in person. (You need to gauge the reading level of the respondent.)
