13 The team leader (provider who attends/admits/transfers or discharges the patient) must be identified or set to unknown. That provider’s call time and arrive time must be valued or set to unknown. The Check can be validated by using the validate button and following the prompts (selecting the appropriate Failure Source and then Confirm Check Validate- Yes/No). The Registry is very case sensitive. ALL CAPS must be used when entering diagnosis along with the “dot”. Our Mission: To protect and improve the health and environment of all Kansans.

26 If a reason exists where a patient was transferred via EMS (including air ambulance) to another facility for example: due to personal preference or malfunctioning equipment; OR if they were first admitted to your receiving facility, then, they are to be included in the registry. 30 Clicking on the link on the Central Site Portal will automatically send an to the Kansas Trauma Program that there were no eligible cases reported for the quarter.
