The following two slides have a drag and drop chart activity to be completed after reviewing the Bright Futures Tool and Resource Kit. Infancy – Newborn (pg ); 3-5 days (pg. 367–369); 1 month (pg ); 2 months (pg ); 4 months (pg ); 6 months (pg ); 9 months (pg ) Early Childhood: 12 months (pg ); 2 years (pg ); 3 years (pg ); 4 years (pg ) Middle Childhood: 5-6 years (pg ); 7-8 years (pg ); 9-10 years (pg ) Adolescence: years (pg ); years (pg ) The suggested responses in the Bright Futures Resource are to be used as a guide. If the family has other concerns, or if the practitioner notes something that is concerning, then the focus should shift to fit the context of the child care situation.  Directions Drag and drop each red “Risk” from the left of the slide into the appropriate age group in the light grey column on the same slide. Drag and drop each green “Strengths and Protective Factor” from the right of the slide into the appropriate age group in the light green column on the same slide. Complete the Social Determinants of Health Age Chart - Anticipatory Guidance Notes Word document. Print slides 2 and 3 as full page slides when complete.
