Developing our integrated localities
The Locality Transformation Scheme is the vehicle we are using to enable general practice to be at the heart of the locality With general practice leadership, integrated provider forums that bring together leaders from the GP locality, community providers (including VCSE), mental health and social care will be critical in establishing new service models… …focussed on meeting the specific needs of the local population As integrated localities become established they will build partnerships with hospital colleagues to design fully integrated pathways that help people stay healthy, well and independent in the community Ultimately our vision is that fully integrated localities will push back the boundaries of what is done in an acute setting and establish the community as the default setting for all of a person’s care.

Community Services service procurement across BNSSG for mobilisation from April 2020 Work with Councils to define plans for recommissioning of community equipment services Continued focus on admission avoidance and enabling discharge Maternity – continued implementation of Transformation Plan Mental Health and Learning Disabilities – service specific schemes, including review of Control Room and Street Triage, 24/7 psychiatric liaison, review of ADHD service, CAMHS pathways, IAPT re-procurement

Healthy Weston – sustainable solution, with community- focused role Planned Care refreshed planned care strategy Greater focus on non face to face out-patients Review ophthalmology pathway Urgent Care Integrated Care Bureau development Development of frailty model Urgent Treatment Centre designation Acute services – standardisation of contract terms between the three acute providers

Medicines Optimisation/Prescribing Improve patient outcomes through better use of medicines and evidence-base care Quality Focus on aligning quality measures and incentives across the system to support positive patient experience Sustained emphasis on reducing healthcare acquired infections Clinical Effectiveness – shifting resources from low value activity to high value activity Primary care BNSSG strategy refresh, workstreams as part of STP to support transformation, resilience and workforce development to deliver GPFV locally
