1. ERMHS Student TIP Therapeutic Intervention Program
A student receiving special education services and has (or will have) counseling services identified in their IEP.
The Case Manager and/or Psychologist will obtain signed referral form and consent for assessment.
Sierra Vista Clinician will assess student and make a recommendation to case manager. IEP team will convene to discuss results of assessment and add Sierra Vista counseling services to the IEP if appropriate. The Clinician will provide the social/emotional goal. Counseling services will begin once identified on IEP.
There is already an IEP in place, the case manager, designated by the school, will change the service provider to Sierra Vista and provide a signed referral form within 72 hours to the Sierra Vista Clinician.
DRAFT
Once the services are updated on the student’s IEP, the Sierra Vista Clinician will open the case up for counseling services. Per school policy, the Clinician may also have access to SEIS.
Clinician will attend all IEPs for the student and provide social/emotional goals as needed.
