Open Enrollment 2018 Employee Benefits
Oct. 16th – Nov. 3rd : Online Self-Enrollment! For the 2018 Enrollment, there are no changes to your insurance options or employee monthly costs. You may enroll in Voluntary Products, such as Term life insurance, Vision, Accident Insurance, Flexible Spending Accounts, Health Savings Account, Cancer Insurance, and Short-term Disability. For more information about how to get enrolled attend one of the benefit group presentations in October 16th at 9am, noon or 4pm in Mingenback Theatre. You have THREE options regarding your enrollment for the 2018 plan year If you do not need to make any changes to your benefits, no action is necessary. All your current benefits will carry over into the new plan year. To make changes or review your current benefits log into the new Online Self-Enrollment (instructions below). Call the BenefitsDirect call center at . Employee Benefit Portal: Beginning Oct 16th, you will be able to enroll online by visiting this site. Self-Enrollment Instructions: Please make sure to have the following information available prior to logging on to the self-enrollment site. Social Security Numbers and birthdates for you and your covered dependents Note: Once you have completed your enrollment, you will not be able to make changes. You can call and speak with a Benefit Counselor to make changes if needed. (855) Follow the On-Screen Login Instructions after clicking Enroll Now on the Benefits Portal HOW TO LOGIN UserID: 1st initial of your first name + 1st initial of your last name + the last 4 digits of your SSN EXAMPLE: John Smith SSN UserID= JS1453 PIN: Date of Birth EXAMPLE: March 16, 1960 PIN= If you choose to change your PIN on the following page, please keep it in a secure location. Once you have successfully logged on, the self -enrollment site will take you step by step, one screen at a time, to enroll in your benefits for the upcoming plan year. Help screens are located at the top of every page of the self- enrollment site. On each help screen you will find product information and tips on how to enroll. A more in-depth outline of the process can be found on page two.

2018 Employee Benefits Open Enrollment Oct. 16th – Nov. 3rd : Online Self-Enrollment! Employee Benefit Portal: Beginning Oct. 16st, you will be able to enroll online by visiting this site. Self-Enroll Process continued below: Employee Data: Verify your personal information. If all data is correct please click “Save and Continue” at the bottom of the screen. Dependents: Verify dependent data. If you need to add a spouse or dependent, please do so here. If your dependents are not on this page, they will not be eligible for benefits. Once dependent data is verified click on “These dependents are correct” at the bottom of the screen. Product Elections: Each product will require an active election. You will need to click one of the following buttons for each product. Detailed Current Election Plan information such as coverage tier, volume or level is available by hovering your mouse over the premium amount of your current elections in the menu bar. Enroll – Click this button if you are enrolling for the first time or are changing coverage after you have selected the appropriate tier or volume of coverage. Waive – Click this button if you do not want to enroll in a specific product. This could be if you are dropping a plan in which you are currently enrolled. This can also apply if you are not currently enrolled and don’t want to be enrolled in the upcoming plan year. Same As Last Year – Click this button if you want to stay in the same tier or volume of coverage. If there is an accompanying premium change, that change will automatically be applied. This button will only populate if you are currently enrolled in a product. Clear All – Click this button if you mistakenly clicked the wrong tier or volume of coverage. Beneficiary: Some Benefits will require entry of beneficiary information. Please enter at least one primary beneficiary on this screen. Primary (and secondary if entered) must equal a percentage of 100%. Once data is entered, please click “These beneficiaries are correct” at the bottom of the screen. Complete Enrollment: Once you are satisfied with your product elections, please click the “Finished” button at the bottom of the page. Process Confirmation Statement: Review your elections. Please verify the elections on the confirmation statement match those that you just elected during your enrollment. Once you have verified the elections, please scroll to the bottom of the page. You will sign electronically with your mouse or finger. Then click “Process Confirmation Statement.” Your Open Enrollment is now complete!
