The expansion counties for this year are in light blue on the map. Counties in darker blue had this D-Snip plan in 2018 and will continue to offer in 2019. We are excited to be partnering with you this year as we expand our coverage of this plan to allow more qualified individuals to take advantage of the additional benefits we will share with you in this Michigan overview

UnitedHealthcare Dual Complete Value Added Benefits and Features: OTC Debit Card: Up to $270/quarter - $1,080/year on an over-the-counter (OTC) Debit Card. Expires annually Dental Coverage: $2,000 annual toward dental care Transportation: Up to 24 one- way rides for health care visits and prescription needs Chiropractic Coverage: Up to 10 chiropractic visits every year Personal Emergency Response System: Members are connected to trained operators in any emergency situation 24 hours a day Vision Coverage: Annual exam and $200 credit every two years for eyewear
