To begin with, let’s take a quick look at the service area in Michigan for As you’ll see on the map, UnitedHealthcare Dual Complete will be active in 20 counties for the calendar year. The expansion counties for this year are in light blue on the map.
Counties in darker blue had this D-Snip plan in 2018 and will continue to offer in 2019.

4. DSNP Michigan Member Benefits
UnitedHealthcare Dual Complete Value Added Benefits and Features:
OTC Debit Card: Up to $270/quarter - $1,080/year on an over-the-counter (OTC) Debit Card. Expires annually
Dental Coverage: $2,000 annual toward dental care
Transportation: Up to 24 one- way rides for health care visits and prescription needs
Chiropractic Coverage: Up to 10 chiropractic visits every year
Personal Emergency Response System: Members are connected to trained operators in any emergency situation 24 hours a day
Vision Coverage: Annual exam and $200 credit every two years for eyewear

Hearing Coverage: Annual exam and $2,000 credit every two years for hearing devices
Foot Care: Four visits per year at $0 copay
Meal Program : Up to 14 meals over seven days following discharge from a facility
24-hr. NurseLine: Members can speak with a registered nurse anytime
