(A) A single clinical diagnosis (e.g., systemic lupus erythematous; SLE) can encompass groups of patients whose disease is driven by distinct molecular mechanisms. This heterogeneity is underpinned by complex gene variant interactions. In this simplified example, there are six equally proportioned groups of patients. Therefore, the predicted response rate to an efficacious drug is at best 17% (one in six). (B) Correlating serial high-resolution molecular profiles with the disease course classify and pairing patients with an appropriate therapeutic intervention strategy. Such molecular phenotyping approaches could provide a new paradigm for establishing ‘molecular diagnosis–treatment continuums’.
