What is the problem we are trying to tackle?
Previous SEN system: Focused too much on disability, problems and negative reputations and chasing resources Has been driven by systems and bureaucracy Has not helped to deliver good life outcomes for many children and young people with SEN and disabilities

What have we learnt that works?

Positive messages from the start
People with complex disabilities can work, be supported to live independently, and participate in their communities Raise aspirations: Employment Independent Living Friends and being part of your community Share what is positive and possible

Transition to adult health services
Professionals must work together with the young person (and their family) to develop a clear and aligned plan The clinical commissioning group (CCG) must co-operate with the local authority in supporting the transition to adult services and must jointly commission services that will help meet the outcomes in the EHC plan.

Transition to Adult Social Care
Under the Care Act 2014, the local authority must carry out an adult care transition assessment where there is significant benefit to a young person or their carer in doing so and they are likely to have needs for care or support after turning 18. For young people with an EHCP the local authority should ensure that the transition to adult care and support is well planned, is integrated with the annual reviews of the EHC plans and reflects existing special educational and health provision that is in place to help the young person prepare for adulthood.
