Your employer has selected a specific benefit amount to complement your medical plan: [$500–$5,000] for each covered hospital admission [(once per calendar year) (once per confinement)] [$250–$1,000] for physician-ordered diagnostic procedures due to a covered accident or illness (once per year) [$500–$2,000] for each covered outpatient surgery, depending on the procedure, to a calendar-year maximum [$250–$1,000] for covered inpatient surgery (once per year) [A travel benefit of $0.40 per mile, for up to three round trips (1,200 miles per round trip), if you meet the following guidelines: You must have a physician’s referral You must have a hospital stay or outpatient surgery The trip must be at least 50 miles one way] [A lodging benefit of $150 per night to lodge a companion during your hospital stay, up to 30 nights] [ENROLLER NOTE: this slide refers to Plan 3 only. 12 Health screening tests include, but not limited to:
Wellness Benefit Based on the plan selected by your employer, this benefit can pay [$50, $75, $100] per calendar year per insured individual if a wellness exam is performed. Health screening tests include, but not limited to: Blood tests Stress tests Colonoscopies Chest X-rays Mammograms A full list of covered tests will be provided in your certificate [ENROLLER NOTE: delete slide if not chosen by Employer or not offered in the situs state In CO, wellness benefit is based on a preventive wellness exam rather than a specific test. In DC and KS, no wellness benefit is offered.] Wellness benefit Employer chooses a benefit amount of $50, $75 or $100 per calendar year per covered individual if a health screening test is performed. The wellness benefit ends when the insured individual’s group hospital indemnity coverage ends.

13 Family coverage options
In addition to employee coverage, you can choose the following coverage options: Employee & spouse Employee & child Employee, spouse & child Who can have it? Spouse coverage Ages 17 to 64 with purchase of employee policy Child Coverage [*] Eligible dependent children newborn until their 26 birthday regardless of marital or student status, with purchase of employee policy. [refer to slide] Eligibility [In IL, child coverage is available newborn until their 30th birthday, if the dependent child is actively enrolled in the military.] In NC, minimum hours worked cannot exceed 30 hours. Employee: Issue ages: no limitation Employees must be legally authorized to work in the U.S. and actively working at a U.S. location Actively at work: Being actively at work means on the day the employee applies for coverage, he/she must be working at one of his/her company’s business locations, or at a location where he/she is required to represent the company. If applying for coverage on a day that is not a scheduled workday, the employee will be considered actively at work as of his/her last scheduled workday. Employees are not considered actively at work if they are on a leave of absence. Spouse: Issue ages: 17-64 Must live in the United States Employee must purchase coverage for himself/herself in order to purchase spouse coverage; coverage for spouse available only if employee is covered Child: Issue ages: dependent child newborn until his/her 26th birthday, regardless of student or marital status Available to the employee’s eligible children, stepchildren, foster children and legally adopted children who are dependent on the employee for support. Employee must purchase coverage for himself/herself in order to purchase dependent children coverage; coverage for children available only if employee is covered Do you have any questions so far? If not, let’s talk about an instance in which benefits may not be payable Employees must be legally authorized to work in the U.S. and actively working at a U.S. location to receive coverage.  Spouses and dependents must live in the U.S. to receive coverage. 14 Coverage effective date
Policy provisions Pre-existing condition† limitation: Unum will not pay benefits for a claim that is caused by, contributed to by or occurs as a result of a pre-existing condition. Coverage effective date pre-existing limitation does not apply [refer to slide] [Enroller, remove this slide if pre-ex has been waived and please keep in mind there are state variations, therefore you must confirm and make the pertinent changes depending on the situs state: Pre-existing condition limitations 12 months prior to the coverage effective date in all states except CO, FL, ID, IN, ME, MT, NM, NV, PA, SD, UT and WY. If sitused in FL, ID, IN, MT, NV, SD or WY display 6 months prior to the covered effective date and 12 months after your effective date. If sitused in CO, ME, NM or UT, display 6 months prior to the covered effective date and 6 months after your effective date. If sitused in PA, display 3 months prior to the covered effective date and 12 months after your effective date.] As with all insurance policies, it’s important for you to understand any limitations or provisions. 15 Why apply now? Advantages of Hospital Indemnity insurance
Hospital admissions can cause serious financial setbacks, due to out-of-pocket costs that are not covered by a typical medical plan. You get affordable rates when you buy this coverage through your employer, and the premiums are conveniently deducted from your paycheck. Hospital indemnity benefits are paid in a lump sum, not as cost reimbursement. You can use the benefits to help pay some out-of- pocket expenses like co-insurance, co-pays, deductibles and other costs associated with hospitalization – or however you choose. A variety of family coverage options are available. [An annual Wellness Benefit is available for all covered family members.] Coverage becomes effective on the first day of the month in which payroll deductions begin.
