ACTIVITIES Participate in the training
Determine that an ICAP is necessary Review Documentation Schedule and conduct interviews Complete the ICAP booklet based on information from interviews and documentation Submit copy to Division of Developmental Disabilities for scoring and calculating funding amount Complete ICAP profiles Discuss results at Individual and Family meeting Facilitate the IPP meeting

INFORMANT GUIDELINES Each informant interviewed individually
One informant from each setting At least 6 months of regular contact with individual One informant must be the individual seeking services

INTERVIEWING Setting up the interview Prepare Yourself Mentally
Prepare your notes Schedule a time Determine the setting Prepare Yourself Mentally Beware of Bias Pre-conceived ideas about the individual Drawing conclusion based on own experiences Asking questions based on own perspective Possible conflict of interest

Two copies of ICAP The Training manual Seat self across from the informant Explain your purpose Consider your posture Be aware of your own personal impact Concentrate on Eye Contact Be aware of your mannerisms

Pace the speaker Allow Wait time Focus on what is pertinent Engage in attending behaviors Ask open questions with minimal encourages Seek Clarification Paraphrase Summarize

Reviewing helps you to: Best manage your time in the interview Clarify information obtained in the interview Increase confidence when scoring the ICAP Biennial ICAPs The individuals most current IPP and any addenda Current Assessments Annual physical report Medications, dosages currently taking Copies of incident reports in the past 30 days Habilitation programs including behavior support plans
