2 THE PARTNERS Hospice Muskoka: Hospice Muskoka has been serving the residents of South and West Muskoka since 1995 The Mission: Hospice Muskoka is a volunteer-driven, community based, not-for-profit organization providing compassionate support to individuals and their loved ones of all ages and backgrounds, who are coping with end-of-life issues Hospice Muskoka has been working to establish a residential program for South Muskoka since In 2012, the Andrew Potts Memorial Foundation revitalized the dream when they approached the board to form a partnership to build “Andy’s House”. Thanks to the support of the community and especially Brock Napier, the Hospice Residence is due to open in 2019.

3 THE PARTNERS On June , Constable Andrew Potts and partner Matt Hanes of the OPP were involved in a car accident while on duty. Tragically, Andy was killed and Matt was seriously injured. In 2006 , The Andrew Potts Memorial Foundation was created to raise money for community projects in Andy’s name. Since 2012, the Foundation has been working tirelessly to achieve a campaign goal of 2.5 million to build Andy’s House, an integrated Hospice Palliative Care Hub. Through their efforts and the generous support of many individuals, groups, businesses and the community, 88% of the funds have been raised. The shovel is ready to go into the ground!

6 A NEW MODEL FOR END-OF-LIFE CARE
The Challenges of Providing Hospice Care in South & West Muskoka: Hospice Muskoka current provides Visiting Hospice Volunteer and Grief & Bereavement support programs throughout South and West Muskoka comprising Bracebridge, Gravenhurst, Township of Muskoka Lakes and Georgian Bay South and West Muskoka has a permanent population of 36,658 and an estimated seasonal population of 63,570 which represents 70.4% of the entire population of Muskoka (“Muskoka Fast Facts” – Muskoka.civicweb.net – general population & employment statistics) 22% of Muskoka’s population are older than 65 years of age versus the provincial average of 15% Permanent population growth is estimated to be about 1% per year The annual income of many year round residents is below the provincial average, with many below the poverty line

7 : Muskoka is served by Muskoka Algonquin Health Care with two hospitals – Huntsville site in the north and Bracebridge site in the south. Both sites have very high Alternative Level of Care levels (patients who occupy acute care beds who could be more economically served in a more appropriate setting) There is an extreme shortage of Personal Support Workers in healthcare across the province, which is particularly evident in rural regions like South and West Muskoka There is a five bed hospice located in Huntsville, one that is scheduled to open in Orillia in 2019, and the next closest is Barrie The only end-of-life care options in South and West Muskoka is to engage Hospice Muskoka Visiting Volunteers, family and friends or pay for private care to be able to stay in the home; be admitted to hospital; transfer to Algonquin Grace Hospice in Huntsville which is projected to reach capacity within the next few years

8 Client and Family The Operational Model for Andy’s House:
Andy’s House, a Hospice Palliative Care Community Hub Residential Respite Care Transition Care Grief, Bereavement and Spiritual Care Client and Family Caregiver Support Volunteer Centre 24/7 Issue and Crisis Response Public and Client Learning Centre Day Hospice The Operational Model for Andy’s House: Existing Programs Expanded Due to Community Hub. New Programs Introduced Within the Community Hub.

9 WHERE ARE WE NOW It is anticipated that it will take 10 months to complete Andy’s House which is anticipated to open in the spring of 2019 if the build is able to break ground in early summer When open, Hospice Muskoka will move its current operations to the site so it will be occupied immediately There are five “Sub-LHIN regions in North Simcoe Muskoka. In 2016, when funding was announced to increase the number of palliative care beds by 200 across Ontario, North Simcoe Muskoka received 10 beds. Two of the Sub-LHINs, Huronia and Couchiching, do not have any residential beds whereas Muskoka (5), Simcoe (10), and Georgian Triangle (6) are served. Consequently, 5 beds were allocated each to the two regions with no beds The Board of Hospice Muskoka has realized that our communities in South and West Muskoka are in desperate need of hospice palliative care alternatives

Hospice Muskoka intends to open 3 beds, self-funded and will apply for the next round of funding for palliative beds (anticipated in 2020) Hospice Muskoka received a community grant from the Township of Muskoka Lakes to host an information and launch of Andy’s House event which will take place on June 21, 2018 Hospice Muskoka is grateful to the Township of Muskoka Lakes for providing an office in the Health Wellness Hub two days per week to help build presence in West Muskoka, engage the community in the plans for Andy’s House and to build a roster of volunteers to assist in the programs and services that will be offered. Hospice Muskoka is in consultation with various agencies to offer all of the program components identified in the business case The presence of Hospice Muskoka in West Muskoka will expand the Visiting Hospice Volunteer and Grief and Bereavement Programs throughout the region

11 “In view of the potential benefit that Andy’s House would bring to South and West Muskoka, Hospice Muskoka requests that the Committee of the Whole recommend to Council that the Mayor and Clerk be authorized to sign a land lease between the Township of Muskoka Lakes and Hospice Muskoka enabling Andy’s House Integrated Hospice Palliative Care Hub to be constructed”
