Updating the CVI/Revising the ENs
Examined updated CVI forms for Wales and Scotland, alongside existing forms for England and Northern Ireland and produced first draft Discussion on drafts of CVI form with ADASS PSI Network (May and September 2016) and amendments made. DH drafted revised Explanatory Notes and sub-group commented and made amendments to drafts. Also shared with ADASS PSI Network Draft documents shared at VISION 2020 Certificate of Vision Impairment Round Table on 13 September and amendments suggested

Main changes The form has been revised to update information and contact details, simplify terminology and ensure it captures the information to support people who may need help from local authorities The form is now more relevant to both adults and children with diagnosis boxes for adults and children rather than separate forms The ‘patient information’ pages highlight the positives of registration and provides more information to adults, children and their families regarding the support that is available to them
