In response to this staffing niche, PMC Insurance Group is offering a broad Workers Comp Program for staffing firms specializing in the medical industry. Target Classes/Operations Hospitals Nursing Homes Home Health Doctors offices Assisted Living Registered Nurses, Nursing Aides and other related services. Class Codes 9040, 8045, 8835, 8833, 8832, 8829, 8826, 8825, 8824 States specific classes are eligible. Program Parameters Minimum Premium $7,500 Available in All States, except MA,PA, AK & HI Guaranteed Cost Loss Sensitive/Deductible programs for larger accounts Admitted A Rated Carrier Submission Requirements WC Acord Application 3 years current valued loss runs Current mod worksheet Completed Supplemental App
