9. Medication
Please complete a medication form and return it by the end of this week if possible.
Final deadline: Tuesday 5th June.
This form is for you to detail the medication your child needs and the frequency of administration.

10. Medication 2
On day of departure, all medication must be handed to Mrs Wilber.
Ensure it is in a clear plastic bag and clearly labelled with the child’s name and DOB.
Travel sickness tablet should be administered by parents on day of departure. If one is needed for the return journey, it needs to be in a clear plastic bag and clearly labelled with the child’s name and DOB.
