3 Why diagnose early? One of the most feared diagnoses.
50% of people with suspected Dementia put off a diagnosis for at least a year. Explain symptoms. Validate thoughts and feelings. Access to treatment to improve or slow down the illness. Advice/support for patients, carers and families.

4 The 5 “p”s Physical illness. Post diagnostic support. Prevent crises.
emPower patients. Prevent significant cognitive decline, reducing cardiovascular risk factors.

5 The missing 178 Prime minister’s challenge. National targets: 66.7%.
Aug 2018: 65% (up 0.5% from previous month.) Dementia gap 178 patients. 2 per practice.

6 How can we help? OPMH Clinical Lead. Raise awareness.
Monthly bulletin. Presentations to various groups. Individual contact. Secure funding.

7 Quality Engagement Framework
£3.70 per patient. 25p per patient for Dementia. Identify a practice GP clinical lead for Dementia. Improve on the current practice Dementia diagnosis rate. Adopt an “inhouse” checklist prior to referral. Engage in/work towards Dementia friendly status.

Identify GP clinical lead: point of liaison. Improve on practice current diagnosis rate: no set target. Try and get 2! Checklist: not onerous. No compulsory use of proformas!

9 Sneak preview GP/patient/carer has concerns.
GP full history, examines, tries to rule out delirium. Quick validated memory assessment. Treats depression if identified. Bloods as per QOF (FBC, B12, folate, renal, liver, thyroid, calcium, glucose) and treat. No need for ECG! Refer to memory clinic if appropriate.

10 How can you increase your rate?
Coming here! Alzheimer’s society in surgery? Buddy: share ideas with neighbours. Care homes. Drugs: anti-dementia or anti-Parkinsons. Environment: dementia friendly? Flu campaign. Guidance: NICE, BMJ online modules. Hearing.

11 Dementia friendly Twilight sessions organised by CCG.
Individual at the surgery. Signs. ?Double appointments. ? Telephone reminders. Chat to Alz soc.or visit website. ?Drop in clinics. “Working towards”.

12 Summary One of the most feared diagnoses. Doesn’t have to be this way.
Early diagnosis has many benefits. The missing 178. Requirements of QEF. Helpful pointers.
