4 5% - 8% Impairment 10% - 13% Impairment Muscle guarding or spasm
Assymetric loss of ROM Non-verifiable radiculopathy NO AOMSI, or Herniated disk w/ no symptoms after non-surgical treatment. 10% - 13% Impairment Significant signs of radiculopathy (e.g. dermatomal pain, sensory loss, loss of relevant reflexes loss of muscle strength, or unilateral atrophy) May be verified by electro- diagnostics or a combination of history & clinical findings or Had surgery, now asymptomatic

5 20% - 23% Impairment AOMSI or Post surgical arthrodesis
AOMSI is defined as: >4.5mm translation of one vertebra on another in both flexion and extension (added together) Arthrodesis is defined as: (essentially) fusion surgery
