1. Fatality: Pedestrian Killed While Crossing Dual Carriageway

INCIDENT
The IP was running across a dual carriageway (speed limit 120kmh), and
made contact with the side of a moving HGV trailer as it passed. The IP
suffered multiple traumatic injuries which resulted in his death. A
pedestrian subway was available which provided a safe route to cross
beneath the road was available at the incident site.
ROOT CAUSES:
The IP failed to use the pedestrian subway provided
The IP took a short cut

Where provided use the correct points for crossing roads.
DRIVERS
Pay particular attention on approach to the school and hospital junctions expect pedestrians to cross the road in this area
Always be on the look out for pedestrians, expect the unexpected
Recommended Actions:
Discuss this alert in toolbox talks and HSE meetings and
Distribute to all contractors
Post on HSE notice boards
Include in site HSE induction
