Logic Model NOT new content!
Content from Action Plan, just rephrased/rearranged Written to help focus, organize, and prioritize actions, outcomes, indicators for success Facilitates “walk-thru” of projects beginning to end

Elements Goal – what we want to be true (NOT what we want to accomplish/carry out!) Outcome – if the goal were true, what would that look like? Background – history, baseline data, what you’ve already done Methodology – what your methods are for making the goal come true - Processes, work that is going to be carried out, actions to be taken, etc. Indicators – of success! What you are going to measure and report out on that show the success of your work/methods. Increase/decrease X by a percent or certain amount, frequencies 4 out of 10 people will XYZ NOT process indicators – “will hold x number of food demonstrations” Process indicators tell us what you are doing, but not the success of your program!
4
Process indicators and how to improve
Original process indicator: THD will hold 12 food demonstrations in 2018 This tells us what THD is doing (holding 12 food demonstrations) but doesn’t tell us if the demonstrations were successful, or even what constitutes success for THD What do we want the community to get out of food demonstrations? New knowledge? Healthy choices, new techniques, changed eating/cooking/habits? Etc! Better indicator: Of 12 food demonstrations, at least 10 will incorporate “food swap” strategies. Even better indicator: 75% of attendees of THD food demonstrations reported they “learned at least 1 food swap strategy they will incorporate” on post-class survey Best indicator: 50% of attendees of THD food demonstrations report on a 3-week follow up survey that they have “used at least 1 food swap during cooking since attending the food demonstration” Not always feasible, but try to write indicators that REALLY speak volumes about the success/impacts of the program!

Public Transportation
Goal: What we want to be true! Tulsa residents choose to use public transportation Outcome: What the goal would look like if it were true. Tulsa residents have safe and easy access to public transportation options Background: History, baseline data In FY16, 2,906,741 people used MTTA public transportation options Methodology: what your methods are - Processes, work that is going to be carried out The CHIP subcommittee group will conduct a study with Tulsa Transit to identify barriers to transit ridership. CoT Traffic Engineering will use this data to prioritize infrastructure improvements near transit stops. Indicators: of success! How will you know if succeed? The number of MTTA users will increase by 5%

Active Transportation
Goal: Tulsa County residents can safely choose active transportation options (walking and biking). Outcome: Tulsa County residents choose active transportation options (walking or biking) to commute. Tulsa County bicyclists and pedestrians are safe in Tulsa County. Tulsa County municipalities adopt plans that facilitate active transportation (walking and biking). Background: In 2016, 1.3% of Tulsa County residents indicated that they walk to work. In 2016, 0.2% of Tulsa County residents indicated that they bike to work. In 2017, 5 Tulsa County municipalities had adopted the GO Plan. In 2016, there were X bicycle/pedestrian crashes. Methodology: In 2018, CHIP subcommittee will work with Tulsa County municipalities to adopt the GO Plan through meetings, data sharing, and engagement with community organizations. In 2018, CHIP subcommittee will identify opportunities to educate motorists, pedestrians, and bicyclists on bicycle/pedestrian safety. In 2018, CHIP subcommittee will conduct traffic calming demonstrations to provide information to citizens and decision makers on how low-cost improvements can increase safety for bicyclists/pedestrians. Indicators: Increase the percentage of people who report active transportation commuting by 10% Increase the number of municipalities who have adopted the GO Plan by 2 Decrease the number of bicycle/pedestrian crashes by 7%.
