Accomplishments “It’s All About You”
52 Leaders Trained, 127 participants completed Outcomes: Decreased ED Visits by 20% / Inpatient by 67% Diabetes Education Program (Lee Health Solutions – LMHS) 132 Community Care Program participants in last year Initial Hemoglobin A1C: 7.5 / 3 month: 6.6 Obesity Clinic: 98 patients in last year (Lee Health Solutions/Lee Community Healthcare) Over past year - 98 patients lost a total of 734 pounds 4 Additional Community Partners: Barkley Place, Interfaith Charities of South Lee, McGregor Clinic, and South Trails Fire District 20 workshops completed as of 5/21/14 with 127 participants completing at least 4 of the 6 weekly sessions. 3 Currently in Progress: Gulf Coast Village, Park Royal Hospital, and Barkley Place

Same day appointments/improved pt. experience/implementing registries SKY Family YMCA Diabetes Collaboration Received Community Transformation Grant (National REACH Coalition & CDC) Works with the Latino/Hispanic Communities (Bonita Springs / Estero) Successfully Conducted the CDC’s Diabetes Prevention Program Family Health Centers of Southwest Florida Several new programs added (Shape It Up, Smoking Cessation) Provided baby showers for low income moms to be Hosted “Diabetes Revolution” event in November Partnership with YMCA - Pre-Diabetes Education
