7. Delivery Process of Impact Aid Forms
Impact aid forms are delivered to each school 1 to 2 days prior survey date
Letters and guidelines are delivered to principals and teachers
Teachers will send impact aid forms home with students on September 20th
Impact aid forms are returned to teachers by September 28th
Teachers will forward tabulation of impact aid forms per class to principal’s office by October 4th
Principal will forward a tabulation of impact aid forms for the entire school to Student Welfare by October 12th
Student Welfare will review impact aid forms and compile report
Impact aid report will be forwarded to the U.S. Department of Education on January 31st
