Student Disability Services Processes
If you need to write your tests and exams in the office of Student Disability Services: Complete the Exam Schedule Form included in your Introduction Letter Package Be sure to submit this form within specified deadlines!

Student Disability Services Processes
The deadlines for submitting your Exam Schedule Form on time are as follows: Midterms must be submitted 7 days before the scheduled test Final Exams must be submitted 30 days before the scheduled exam

IMPORTANT TO REMEMBER: If a situation arises where you must change the dates of either your midterm exam or final exam, You must complete the Exam Change Form

For Your Information After you have submitted your exam schedule, your exam information is entered into an exam database This database is designed to report exam activities each day that exams have been scheduled

For Your Information The Exam Coordinator uses the information in the exam database to: Inform your instructor of your exam accommodations Send instructions on delivery of your exam to the Student Disability Services And to schedule proctors for your exam

REMEMBER: Only the tests/exams where you require accommodations should be scheduled with Student Disability Services All tests/exams are to be scheduled on the same date/time as the rest of the class. It is your responsibility to ensure the date, time, course codes and instructor information is accurate on the Exam Schedule Form

REMEMBER: It is also your responsibility to contact the Exam Coordinator of any changes to your schedule. This can be done: Through Phone ext. 3463 Or by submitting an Exam Change Form

REMEMBER: If you fail to inform the Exam Coordinator of any changes to your exam schedule and you do not show up for a scheduled exam, you will be charged a Proctor Service Fee of $25.00 for each missed exam

REMEMBER: report this dropped course either by email
If you drop a course and have already scheduled tests/exams for that course, you must notify the Exam Coordinator IMMEDIATELY! report this dropped course either by or by completing the Notice of Course Change Form (also included in your Introduction Package)

What you need to know when you are writing an exam with Student Disability Services……. All students are responsible to comply with the Examination Policies as laid out by the University of Windsor : Senate Policy E3

Student Disability Services has trained proctors who are responsible for supervising all students who write tests/exams in our office These proctors are very familiar with the Rules of Conduct and Examination Policies as seen on the web page

What if I’m unable to attend the exam?
Call the proctor extension ext 3477 as soon as you know Get documentation to support the reason you are unable to attend the exam (doctor, funeral home, etc. – must be a serious reason – see senate by-laws for clarification)

Advise your professor – (preferably by ) so you have a record of absence and you can receive further instructions if necessary Advise your disability advisor – (preferably by ) in the case where it is disability related

Reschedule an exam due illness cont’d
Confirm how your professor would like to re-assess you and reschedule through our office where applicable – (Use exam reschedule form)
