Medication errors, confusion, and communication breakdowns lead to:
1.3 million injuries due to medication-related errors in 2007
98,000 medication-related fatalities in 2007
Decreased patient safety, satisfaction, and ability to manage their health at home
Increased provider liability
Opportunities for Improvement:
Patient satisfaction and self-efficacy
“Meaningful use” standards for providers
