Steps to create a form
Log into your employee portal
Select “Office 365”
Login using the same credentials as the employee portal
Select “Forms”
Select “New Form”
Select “Add Question”
Select the type of question
Continue until the form is ready
Once finished, select “Share” top left
You can either the link or copy and paste the link

2. Retrieving the responses later…
Log into employee portal
Select “Office 365”
Login using the same credentials as the employee portal
Select “Forms”
Select the form which you ed for response
Once open, there is a second tab labeled “Responses”
Here you can retrieve all of the statistics of the responses and it provides an option to download in excel
