5 Team-based Approach Designed and managed with a particular emphasis on team-based care and coaching The design encourages a proactive focus on health and life balance. To explore, define, and reach your customized health and well-being goals We feel it is extremely important that each care team member knows how you are doing. The team works very closely with each other to know and understand you, and how we can each contribute to providing the highest quality of care.

6 Team-based Approach Each care team includes a doctor, nurse practitioner, registered nurse, health coach, and medical assistant. (18 team members) Patients are the most important team members. The information from patients’ visit is available to every member of patients’ care team, including patients. Dissolves the traditional barriers between doctor and patient We feel it is extremely important that each care team member knows how you are doing. The team works very closely with each other to know and understand you, and how we can each contribute to providing the highest quality of care.

7 Collaboration Building a collaborative relationship between patients and their care team The highest degree of communication and collaboration between patients and the care team  Custom-designed environment Exam rooms are clustered around a multidisciplinary team work area to enhance operations by providing space for collaboration. Enabling patients to connect more directly and deeply with the management of their own health and chronic illness The Ambulatory Practice of the Future, a primary care clinic adjacent to Massachusetts General Hospital, was designed to support collaboration among providers, transparency of information and connectivity, says Coull. The design features clusters of exam rooms around a central multidisciplinary team work area and shared multipurpose space where clinicians can consult with one another. It is intended to serve as a prototype practice for developing a patientcentric culture of care. We feel it is extremely important that each care team member knows how you are doing. The team works very closely with each other to know and understand you, and how we can each contribute to providing the highest quality of care.
