Other bills taken as proof of address: current lease, property tax notice, homeowner’s insurance bill, mortgage statement, current PeachCare eligibility documents

Fill out the Proof of Residency form, then…
You and the person you are living with get the form notarized.
The person you are living with must bring or send a bill with a physical address as proof of their address.
Make sure the bill and the form have the same physical address.
