List of current medications This should include all prescriptions, over-the-counter medications, and herbals What is the dosage taken? How frequently do you take this medication? How long have you been taking this medication? What is the purpose of the medication? What monitoring is required for each medication?

4 UPDATING MEDICATION INFORMATION
Information about your medications should be reviewed with you at the following times: 1. Clinic visits 2. ER visits 3. Hospitalizations 4. Transfer of care to another provider or organization

5 OTHER INFORMATION YOU SHOULD BE AWARE OF
What are the side effects of these medications? Are there any special instructions for taking each medication, i.e., special foods or times or activities which might effect the benefits of the medication? With each new medication added, should you continue to take your previous medications? Are there other medication names that sound just like or look just like this one?

6 WHY????????? “Medication Reconciliation” is one of the efforts to reduce the number of medication errors which occur world-wide every day. It is important that every person be responsible for their own health care and what happens to them. This is a way to help all of us be more health conscious.

7 WHERE DID THIS COME FROM
There are eight (8) national patient safety goals. All medical care entities incorporate the national patient safety goals into their facility quality of care programs. The 8th goal reads: “Accurately and completely reconcile medications across the continuum of care.”

8 8th NATIONAL PATIENT SAFETY GOAL
This goal has been interpreted to mean: Review and document with the patient a complete list of each patient’s current medications upon admission to the organization (clinic, ER, or care facility, i.e., hospital or nursing home). This information shall be communicated to the next provider of service upon referral or transfer within or outside the organization.
