Chest radiograph showing extensive bilateral interstitial infiltrates with ground glass shadowing. Black African HIV positive male, CD4 count 200 cells ×106/l, presented with 2 months of increasing dyspnoea, non-productive cough, and fever. Thought initially to have Pneumocystis carinii pneumonia but bronchoscopy negative and patient failed to respond to high dose co-trimoxazole.
