2/24/ :19 PM MATERNITY RECORD Started by MW by 10 weeks of pregnancy Full medical / pregnancy and social history taken at Booking and any problems identified & documented All maternity care is documented within the Electronic record throughout the pregnancy episode of care In the next few weeks the revised version of the National Maternity Record will be uploaded into the EPR (electronic patient record ) of the Live system. Within that revision of the record - GIRFEC has been incorporated into the Maternity record keeping. This includes the GIRFEC lead professional, health plan indicator, Concerns / risks & significant factors and a chronology of significant events. (screen grabs that follow further demonstrate this function) Postnatally – the care of mother and baby is handed over to the Public health practitioners at 10Th day – an electronic link has been created from the EPR baby Discharge documentation to the Child health record – which is activated at transfer of care. The function was designed before multi-agency requirements. 2
