OB 2/3 Evaluation of a Cervix
Transabdominal (TA) evaluation of the cervix should be attempted on every OB2/3 patient <26wks & TA is suboptimal*? Transvaginal (TV) evaluation of the cervix is necessary: No Yes Prior exam w/ normal cervix**? No Yes Yes <32wks & any of the following are true? ant low-lying placenta & hx c-section TA evaluation is abnormal*** hx of cervical incompetence uncertain placental-cervix relationship**** hx of preterm delivery (<37wks) abnormal cervix on prior exam hx of cervical procedure (LEEP, cerclage, etc.) preterm labor during current pregnancy No *unable to see and/or cannot be reliably measured **outside facility is okay, but a report and/or images needed to verify ***funneled and/or shortened (<3cm) ****if placenta is low lying (<2cm), but sonographer doesn’t think TV is beneficial, then review with rad for approval No TV exam is not necessary
