What is Coordinated Entry?
Coordinated entry is a process developed to ensure that all people experiencing a housing crisis have fair and equal access and are quickly identified, assessed for, referred, and connected to housing and assistance based on their strengths and needs.

Goals Create a system that streamlines access and referrals to services and housing Ensure that all people experiencing a housing crisis have fair and equal access Use standardized tools and practices Prioritize homeless assistance for those with the most severe service needs and highest vulnerability Better serve client needs

How does it work? Know everyone by name Housing First
By-name list Housing First Prioritization Fair and equal access Standardized assessment Know everyone by name The community uses a by-name-list of persons experiencing homeless to coordinate and prioritize referrals to housing and services Housing First Prioritize based on need and vulnerability, not on whether someone is deemed to “ready for housing” Fair and equal access Standardization of assessments and protocols, and multiple points of access lead through the same process, engage persons through outreach to meet them where they are at
