4. ICM Referral and Enrollment
ICM referrals from staff and/or community members are given to the Program Manager
Program Manager distributes referral to ICM Case Workers
ICM Case Workers enroll clients
Complete ICM Assessment using Outcome Matrix
Update or create ETO service plan
Discuss needs and best course of action with client

5. ICM Case - After Enrollment
All direct service staff meets biweekly with to provide updates and coordinate services
ICM Case Workers track the 3, 6, 9, and 12 month ICM assessment by setting reminders on the shared Outlook calendar
Cases are reviewed when opened and when closed by Program Manager

6. Case Study
Client “Sarah” was referred to ICM by her R&P Case Worker (CW) to assist with a medical issue and acculturation. Sarah was also receiving services from the Employment department. Sarah’s medical issue was resolved but Sarah started missing work and looking tired when she went to work. The employer called IRC and Sarah’s Employment Specialist relayed the employers concern to the ICM CW. When the ICM CW met with Sarah she said she was very tired lately and wasn’t able to sleep because she was thinking all the time. They discussed possible solutions and Sarah decided to meet with a therapist. Sarah’s ICM CW connected her to a mental health specialist that speaks her language and coordinated ongoing appointments. The ICM CW discussed Sarah’s appointment schedule with the Employment Specialist and the Employment Specialist coordinated Sarah’s time off with her employer. Sarah is working and seeing a therapist once a week.
