(Left) High-definition white light endoscopy of the distal sigmoid colon in a patient with longstanding quiescent ulcerative colitis. The mucosal area highlighted shows a subtle depressed irregularity. (Center) Methylene blue 0.1% chromoscopy has been applied to the mucosa. A clearly defined flat, depressed lesion is delineated (Paris class 0–IIa + IIc). (Right) Crystal violet (0.05%) chromoscopy has been applied to the lesion and high-magnification imaging acquired at the area of central depression highlighted. The magnification imaging (×100) shows a Kudo type V(a) crypt pattern. Final diagnosis was of a high-grade dysplastic intraepithelial neoplastic lesion.
