1 Adding further documentation after Appraisal is completed
for Appraisees (Primary and Secondary Care)

2 Additional Documentation after Appraisal (1/2)
When Form 4 has been signed off – the appraisal is marked as completed and all appraisal forms and documents submitted will be archived No changes to the appraisal forms or documents can be changed beyond this point However, it is possible to request for Additional Documentation to be added to the appraisal

3 Additional Documentation after Appraisal (2/2)
Form 4 will need to be unlocked, and the request for this will need to be from the Appraiser to the local Admin team When Form 4 has been unlocked and Additional Documentation enabled, you can upload further files as required to the interview But Form 4 will need to be signed off again, as the Appraiser may need to edit accordingly

4 When Form 4 has been unlocked, you will receive email confirmation
Navigate to Interview details (My Appraisals > notepad icon), and you will see a new tab for “Additional Documentation” Upload any additional files as required; or Link newly added documents from General Library Items already submitted to interview will not be available for selection Click “Notify Appraiser” when finished Appraiser will receive , requesting them to review the new files, edit Form 4, and re-forward to you for sign off again
