The Internal Medicine Center is an internal medicine residency based clinic that serves ~10,000 active patients in an urban, underserved area (keep expanding about population, trainees, etc.) If doing multi-center study, describe each site

Study Population Number of subjects enrolled (including sub-groups if enrolling at multiple sites): Age of patients that may be enrolled Location of patients that may be enrolled (outpatient/inpatient/other) Any specific population characteristics (e.g. specific comorbidities, vulnerable populations, etc.) Inclusion/exclusion criteria Study timeframe

Study Design Type of study design (e.g. cohort study, cross-sectional, randomized controlled trial, etc.) Approach chosen for assessing the impact of the intervention (statistical models, survey results over time, etc.) including the assessment of any unintended impact of the intervention Approach used to establish whether the observed outcomes were due to the intervention(s). (Was there a control group?)

Variables Collected/Measures
Give brief overview of variables collected, noting if they were retrospective or prospective (they will all be one or the other) You can list them all and highlight the ones you want to focus on, or list the ones you want to focus on only, noting that there were other variables collected (it will depend on how many there are) Discuss how dependent (outcome) and independent variables are categorized and measured.

Data analysis What statistical methods/models will be used to draw inferences from the data as well as look for variation in the data, including the effects of time? See Mike Oravec for help during design phase of study. Power analysis (if necessary) - Ensure that your study has adequate statistical power to answer research question (if applicable – Discuss with Mike Oravec during design phase).

Strengths surrounding study design, population, filling of literature gap, etc. Limits to the generalizability of the work Factors that might have limited internal validity such as confounding, bias, or imprecision in the design, methods, measurement, or analysis Efforts made to minimize and adjust for limitations
