Nurse and or Consultant Triage of Referral
Multidisciplinary Assessment Session for New Patients and / or onward referral to
Type 1 or Type 2 Education sessions
Secondary Care - not part of the routine service but for complications e.g. nephrology
Follow-up sessions as required see pathways
Discharge
