Insert own logo Adverse events: Safe to report, safe to learn When something goes wrong let’s report it and learn from it. We support a culture of openness where adverse events are identified, reported and managed and everyone is responsible for taking action to learn from them and prevent them. Report An adverse event is the term used to describe an event that could have caused, or did result in, harm to people, groups of people or the organisation. Mistakes do happen and we want everyone to feel able to report both near misses and events that cause harm. We have policies and systems in place for reporting and managing these events and staff are encouraged to report – NHS board to insert links for local information. Review We have a process to review all reported events to ensure any lessons are learnt and improvements are made for the future. We will engage staff in this process and provide feedback on the outcomes. Learn Learning from what has gone wrong and promoting best practice is the way we can all make a difference. If it’s not reported we can’t learn and then today’s near miss may become tomorrow’s harm. Remember, safety is everyone’s business.
