Trauma Charting Process Reminders
For ALL traumas (Alpha, Bravo and Limited) All traumas must be charted on the paper Trauma Flowsheet through patient discharge OR Upon receipt of admission orders, then you may transition to EPIC – if you transition to EPIC, in the narrative write: “Admission orders received, see EPIC” All original Trauma Paperwork is to be turned in to the A.A.

Trauma Charting Audits
Audits are now being done weekly Last week our quality charting took a nose dive going from last month’s increase of 89% down to 71% Missed charting ranged from the ever popular I & O Totals to just an abrupt stoppage of charting Possible causes this past week: Decrease in staff Decreased ability for through put – holding admits Periods of high acuity /high volume New process of charting on paper flowsheet until discharge or RECEIPT of admission orders A good way of telling that you are writing the right stuff in the narrative is if you (or someone unrelated to that patient’s care) in 5-7 years at a deposition could look back and be able to ‘tell the story’ of this trauma patient and know what happened to them here in the ER/Trauma
