Background WCCIS provides a “Once for Wales” solution
Bridgend County Borough Council 1st implementing organisation Western Bay Optimal Model – integrated health and social care Bridgend Community Resource Team (CRT) based in Trem Y Mor: Structure Common Access Point (CAP) & Multi Disciplinary Triage (MDT) Therapies – Reablement, Occupational Therapy, Physiotherapy, Speech & Language, Dietetics Acute Clinical Team (nursing element) Social Work Assistive Technology Dementia Support Pharmacy Purpose Supporting people to remain at home Minimising unscheduled admissions to hospital Bridging the gap between inpatient/community care and supporting seamless discharges Focus on enabling and reabling individuals to remain as independent as possible
