Refers to 7 areas in the hospital setting which, if not appropriately monitored, create hazards and risks to the safety of patients, employees & physicians.

9. Code: Fire Alarm
FIRE -Can be activated by any employee or physician by pulling a fire alarm pull station.
R-A-C-E Acronym
R = Rescue anyone in immediate danger.
A = Activate the Alarm – Pull the Fire Alarm Pull Station, Call 911 (Alexian) Call 555 (Adventist) Call 911 (Off Sites)
C = Contain the fire by closing doors
E = Extinguish fire or evacuate

P = Pull the Pin
A = Aim at the BASE of the fire
S = Squeeze the trigger
S = Sweep side to side

11. Code: Severe Weather
Overhead Page for Tornado and/or Severe Weather - close all drapes/blinds, move patients away from windows
Severe Weather Alert: Potential for the development of thunderstorms which may produce hail or damaging winds –
Tornado Warning: will be announced when a tornado is either occurring or imminent based on radar reading
Return to normal activities as “All Clear” is called

12. Code: Security Emergency
Security Assistance Needed for Violent Incident
Call 555 (Adventist) 911 (Alexian) for help or use duress button if your area has one and then also call emergency number if possible
DO NOT Endanger Yourself!
Your Security Team will provide a response for violent or potentially violent situations. Security Officers receive training in Non-Violent Crisis Intervention and are well equipped to provide assistance in most violent situations.
Return to normal duties as “All Clear” is called

13. Code: Utility Failure
Will be Paged out as “Phone Out, Power Out, etc.”
Follow “Utility Failure” policy in the Emergency Preparedness Manual (hard copy or online)
Directions will be given from Facilities staff.
Return to normal activities as “All Clear” is called.

14. Code: Hazardous Spill Hazardous Material Spill
Stop activity that may cause contact with substance. Protect all persons – keep area clear
Dial 555 (Adventist) 911 (Alexian) to report Spill
Return to normal activities as “All Clear” is called.

15. Code: Infant/Child Abduction
Child Abduction – An infant/child is reported missing or is known to have been abducted.
Switchboard announces location of abduction, age and sex of child, and last known location of abductor overhead.
Secure doors and seal off the unit until the “all clear” is given.
Associates respond to exterior doors of facility and discourage anyone from exiting the hospital. If individual insists on leaving, get description and call emergency number.
A description may be announced of infant/child or abductor, but it is important to question everyone.
Associates should NOT physically engage the suspect. Associates should call emergency number to report (Adventist) Call 911 (Alexian)

16. Code: Patient Elopement
Patient Elopement can be activated for a possible elopement of a patient, who does not have the right to leave.
Patients who are certified and petitioned
Patients who have otherwise been determined to be unsafe to leave
Age/location/gender/clothing and description will be given.
Call 555 (Adventist) Call 911 (Alexian) to report
All hospital associates check entrances and building perimeter.
A description may be announced of the patient, but it is important to question everyone.
Associates should NOT physically engage the patient. Associates should call emergency number to report.
Return to normal duties as “All Clear” is called.

17. Code: Armed Intruder
Law Enforcement Assistance needed for individual brandishing a weapon with intent to use.
Dial 555 (Adventist) 911 (Alexian) and give location
Immediate Area - RUN/HIDE/FIGHT
Associates should not attempt to provide medical care to victims.
Return to normal duties as “All Clear” is called.

18. Weapons on Property
The Illinois Concealed Carry Act prohibits weapons on healthcare facility’s property (may be properly stored in vehicle).
If any individual is noticed to be in possession of a weapon within our facilities, associates should contact the Security Department to inform them.
Law Enforcement Personnel will be allowed to carry their weapons in accordance with their policies and procedures.

19. Code: Emergency Operations Action Plan (legacy Adventist)
Standby: initiated when a potential disaster has been identified, the extent of the hospital response is unknown at this time - normal facility operations continue
Level 1: The extent of involvement and the current staffing in the ED is able to handle the increased patient volume
Level 2: Actual incident has been identified resulting in an increased influx of patients – all hospital associates will remain on duty until released by Incident Command
Return to normal duties as “All Clear” is called.

20. Code: Emergency Operations Action Plan (Legacy Alexian)
Alert: Disaster possible; increase preparedness
Standby: Potential disaster probable; ready for deployment
Call Out: Disaster exists; deploy
Stand Down: Disaster contained, resume normal operations
Return to normal duties as “All Clear” is called.

21. Bomb Threat
Call 555 (Adventist) 911 (Alexian); if possible have someone else call while you stay on line.
Try to prolong conversation and do not hang up. Listen to all noises:
background noise, tone of voice, male/female, what age, unusual words or phrases, accent
Give all information to operator.
If a threat is received in writing, handle as little as possible and keep all contents. Call emergency number immediately to report.
Decisions on how to proceed will be made by Incident Command along with local law enforcement.

22. Drills We are required to do drills on emergency codes.
Codes are not announced as ‘drills’
Drill participation is not optional – you must respond.
Use the department’s Emergency Response Manual (hard copy or online) to identify your role via job action sheet.
Hospital and/or outside observers are placed to observe associate response during all drills.

23. Security Services
Contacting Security – Security can be contacted 24/7 for assistance.
Escort on property – Your security team can provide an escort for safety or way finding.
Motorist Assist – In most cases your Security Team can provide jump start or lockout assistance. If other vehicle assistance is needed your Security Team can provide information on towing.

24. Access/ID Badges
All associates must wear ID badge at all times and they must be above waist and below shoulder.
If you need additional access, speak with your Director.
Do not enter areas you do not have access to without escort or permission.
Wait for controlled doors to close before leaving to prevent “piggybacking”.
Conceal codes when entering. Do not share badges or codes with anyone.
