Slide 17 – One of the specific programs that we administer through the division is the Hearing Aid Assistance to the Aged and Disabled program (HAAAD). The program offers a $100 reimbursement toward the purchase of a hearing aid. The eligibility standards are the same as PAAD. The individual must also have a doctor’s attestation for the need of a hearing aid and a paid receipt of the hearing aid.

Senior Gold Prescription Discount Program
Prescription Costs Senior Gold Prescription Discount Program Co-payment is $ % of the remaining cost The catastrophic cap is $2,000 if single, or $3,000 if married Once catastrophic cap is met, the co-pay is just $15 for the remainder of the eligibility year

Lifeline Utility Assistance Program
Utility Costs Lifeline Utility Assistance Program Annual benefit of $225 to offset natural gas and/or electric heating costs Utility customers receive as a credit on the utility bill; tenants get a check made out to them and landlord if utilities are included in the rent Supplemental Security Income (SSI) recipients already receive Lifeline benefit included in their monthly check Slide 10 – Another program through the division is the Lifeline Utility Assistance Program. The Lifeline program has an annual benefit of $225 to offset and eligible individual’s natural gas or electric heating costs.

Low Income Subsidy (LIS)
Medicare Costs Low Income Subsidy (LIS) Known as “Extra Help”, pays Medicare Part D premiums, lowers co-pay amounts and eliminates the donut-hole and any late enrollment penalties for eligible individuals The average person saves $3,900 each year in prescription costs Slide 11 – To be eligible, an individual needs to be a utility owner or a tenant in which their utilities are included in their rent. The requirements and income standard is the same as PAAD ($27,189 for a single individual and $33,334 for a married couple).

Medicare Savings Programs
Specified Low-Income Medicare Beneficiary (SLMB) and Qualified Individual (QI-1) programs Pay full or partial Medicare Part B premiums for individuals entitled to Medicare Eligibility is income between 100% and 135% of Federal Poverty Level and resources not exceeding $7560 for single and $11,340 for married Slide 15 – The two Medicare Savings Programs administered by our division are the Specified Low-Income Medicare Beneficiary (SLMB) and Qualified Individual or (QI1). These programs pay an eligible individuals Medicare Part B monthly premium. In 2018, the annual premium amount that individuals must pay is $1,608; which for the population that we are speaking of is ten to 15 percent of their total annual income.

This chart gives you an idea of the savings that an individual can receive through the benefits of our programs. The possibility of $4,500 in savings for an individual who may only have 20 to 30 thousand dollars of annual income is a tremendous help to this population.

Caregiving Adult Day Services Support for individuals with Alzheimer’s Diseases or other related diseases by offering caregiving support and activities designed to engage and interest those with cognitive loss from dementia 36 contracted centers in 18 counties Must have a diagnosis of a permanent, progressive dementia (Alzheimer’s, multi-infarct dementia, Lewy body, etc.) Slide 19 – Here are some other programs that we administer with the Division of Aging Services. Adult Day Services is caregiving support for individuals with Alzheimer’s or other related diseases.

Caregiving Health and Wellness
A statewide infrastructure in partnership with local providers for the delivery of evidence-based disease prevention programs such as: Chronic Disease Self-Management, Diabetes Self-Management, A Matter of Balance, Project Healthy Bones, and HealthEASE Slide 21 – Our Health and Wellness unit has prevention programs such as assisting with chronic disease and diabetes self-management, along with a matter of balance and project health bones to keep seniors fit.

Jersey Assistance for Community Caregiving (JACC)
A long-term program to support individuals with nursing home-level needs, but who live at home Coordinating agency and care managers in each county State nurses visit to evaluate if the individual needs a nursing home level of care Discontinues after Medicaid starts Slide 22 – Jersey Assistance for Community Caregiving program JACC supports individuals who still reside at home in their community but have nursing home-level needs.

The Office of the Public Guardian
Guardianship The Office of the Public Guardian Provides guardianship services to incapacitated New Jersey residents over the age of 60 Handles medical, social, financial, legal and all other aspects in the lives of clients, including work after guardianship ends Uses services of social workers, nurses, attorneys, paralegals, accountants, bookkeepers, investigators and administrative staff to handle all aspects of the life of the client Slide 23 – The Office of the Public Guardian provides guardianship services to New Jersey residents over the age of 60 who are incapacitated. They handle any and all medical, financial and legal aspects of the individual’s life.

Adult Protective Services (APS)
Guardianship Adult Protective Services (APS) A Federally mandated crisis Intervention program that responds to reports of alleged abuse, neglect or exploitation of vulnerable adults in the community Provider agencies in all 21 counties: County Boards of Social Services, Area Agencies on Aging, and private non-profit organizations 7,500 clients were served during the last full reporting year Slide 25 – Adult Protective Services responds and investigates reports of alleged abuse, neglect or the exploitation of vulnerable adults. They are based in all 21 counties.

Program of All-Inclusive Care for the Elderly (PACE)
Managed Care Program of All-Inclusive Care for the Elderly (PACE) An evidenced-based managed care model that provides all preventive, primary, acute and long term services to the frail and disabled elderly Provider agencies are paid a capitated monthly payment by Medicare and Medicaid and assume “full risk” for the cost of services for each participant Participants must be aged 55 and older and able to live safely within their home at the time of enrollment Slide 26 – Program of All-inclusive care for the elderly, PACE program, provides managed care for preventive, primary, acute and long term services to disabled elderly. Individuals must be age 55 or older and have the ability to live safely within their home. PACE also provides community and assisted living based services.

Statewide Respite Care Program
Support Statewide Respite Care Program Short-term or intermittent relief of uncompensated caregiver (family, spouse, friend) who does daily basic care Sponsor agencies in each county; Complete applications and arrange services Sliding scale based on care recipient’s/couple’s income Discontinues after Medicaid starts Slide 27 – The NJ Statewide Respite Care program offers both short-term and intermittent relief to an uncompensated caregiver such as a family member or close friend who provides basic daily care for another. The monetary relief is based on a sliding scale of the recipient’s income.
