Quality of Life Workgroup
Cancer Plan Overarching Goal: Survivorship & End-of-Life Care Collaboration on the work going on related to nutrition and physical activity, survivorship & end-of-life care, and cancer directed therapy & supportive care Promote the activities of our many partners working to improve survivor health Stowe Weekend of Hope Palliative Care Conference Supported & promoted the Vermont Cancer Survivor Network comprehensive resource website for survivors Kolina Orchard Retreat for Young Adults Impacted by Cancer

Survivorship Care Plan Taskforce
GOAL 15: Implement survivorship care plans Implemented a survey of Vermont Commission on Cancer Centers Survey questions asked about status of Survivorship Care Plan (SCP) implementation and successes and challenges All 5 Survey Centers have begun implementing SCPs Findings identified successes and barriers VTAAC will be working with centers to share solutions and best practices

Commission on Cancer Taskforce
GOAL 12. Improve access to optimal cancer directed therapy among Vermonters. Goal: Support CoC centers in meeting standards: Clinical care Prevention & screening projects Quality Studies QI Projects Collecting info from CoC centers (Summer/Fall 2018) VTAAC to develop recommendations and resources to support CoC projects (January 2019)

Prevention & Detection Workgroup
Cancer Plan Overarching Goals: Cancer Prevention & Early Detection Collaboration on the work going on related to breast cancer, cervical cancer, HPV, colorectal cancer, skin cancer, oral health and more Goal: Present individual workplans and discuss how the group can support its members Result: Formation of a Taskforce or sharing of resources Men’s Health Taskforce, along with Colorectal cancer taskforce, breast cancer, cervical cancer, HPV, etc all fall under this taskforce. However, for the sake of time and since we have a wonderful research panel coming up at lunch where many of those topics will be covered, I’m going to highlight a few projects this year. Conferences – I make note of these events because not only do they promote this work, but they are catalysts for new partnerships in prevention & detection as well as quality of life.

HPV Stakeholders Roundtable GOAL 5. Prevent HPV infections among young Vermonters. Community organizations convened on the topic of HPV vaccination to leverage the strengths of our partnerships and to identify shared objectives around HPV vaccination Goal is to increase HPV vaccination and areas requiring more attention Message: HPV vaccination is cancer prevention The vaccine is safe and effective The HPV vaccine is for boys and girls The HPV vaccine is most effective when given at ages 11 and 12

Lung Cancer Screening Taskforce
GOAL 10: Increase Early Detection of Lung Cancer Among Vermonters 2017 Survey of Vermont’s lung cancer screening facilities Lung cancer is the number one cause of cancer death in Vermont The majority of lung cancers are diagnosed in late stages Barrier: Lack of awareness among primary care physicians regarding screening guidelines. Next Steps: Lung Cancer Screening Summit – November 15, 2018

Skin Cancer Taskforce Education to schools via school nurse bulletin
GOAL 6A: Reduce Exposure to UV Radiation from the Sun & Sun Lamps Education to schools via school nurse bulletin Coordinated education to Sugarbush Resort Employees around the importance of skin cancer prevention and early detection Support the work of Vermont Cancer Plan Implementation Grantee IMPACT Melanoma to increase skin cancer prevention behaviors in Vermont Media opportunities in Summer 2018

Bi-State Primary Care Bi-State supports 11 Vermont Federally Qualified Health Centers (FQHCs) Project focuses on: Increasing cervical, breast and colorectal cancer screening in FQHCs Development and implementation of a FQHC cancer data dashboard Implementation of cancer screening QI initiatives in Vermont FQHCs Vermont Department of Health

Project: Increase colorectal and lung cancer screening working with SVHC primary care offices Primary Care Provider education and practice-level quality improvement activities Patient awareness education Development of provider reminder prompts in Primary Care Provider office electronic health record (EHR) Vermont Department of Health

Take Action Addressing cancer requires a lot of work on the part of many Hundreds of people & organizations help to implement the Vermont Cancer Plan Everyone can play a role in the fight against cancer
