ONE HOUR RESTRAINT PHYSICIAN RE-EVALUATION – Nurse’s Role
Joint Commission Standard PC requires a Provider to evaluate and re-evaluate the patient who is restrained. The in-person evaluation must be conducted within one hour of the initiation of restraint of violent or self-destructive behavior. The evaluation must address the following criteria: An evaluation of the patient’s immediate situation, patient’s reaction to the intervention (restraint), the patient’s medical and behavioral condition and determination if the restraint needs to be continued or can be terminated. The Primary Care Nurse and Facilitator should assist with ensuring the re-evaluation is completed within the one hour timeframe A green star icon will display on the EDTB when the restraint order is placed as a flag that the re-evaluation is needed The provider will complete and document the re-evaluation within one hour of the initiation of restraint. When completed the provider will remove the green star from the EDTB The Facilitator will follow up with the Provider to ensure the re-evaluation standard work is completed THE ED PROVIDER WILL COMPLETE THE RESTRAINT ONE HOUR RE-EVALUATION Select ED Provider Note – Restraint One Hour Re-Evaluation Provider will Complete Template to Meet JC Requirements Restraint orders have an age specific time limit: > 18 years old will need to have a new restraint order every 4 hours 9-18 every 2 hours < 9 years old every hour When the decision is made to terminate the restraint, the Provider will need to discontinue the order in Soarian Clinical
